If not for sale, indicate “NFS.” Please use the attached forms. They may be photocopied. Senior Awareness Day Thursday, May 28, 2015 Micke Grove Park Lodi, CA Day of the Event Set-Up: Art work must be delivered to the Art Exhibition area (next to the bingo area / Mokelumne Shelter) in Micke Grove Park between 6:45 a.m. and 7:30 a.m. on Thursday, May 28, 2015. A limited number of panels are available for display. If you have your own easel, display table or chair we encourage you to bring them. Pick-Up: Art work MUST be picked up between 1:00 and 1:15 p.m. the day of the event. Work may be released to a friend if prior written authorization is given by the artist, mentioning the artist and the person authorized to pickup. LIABILITY Neither the Human Services Agency, nor the San Joaquin County Department of Parks and Recreation, nor their representatives, nor any member of the above names will be responsible for any injury, loss or damage that may occur to the exhibitor or the exhibitor’s property for any cause whatsoever. The exhibitor, on signing the application, expressly releases the aforementioned from any and all claims for such loss, damage or injury.
