Do dnia 8 kwietnia 2015 roku studenci Uniwersytetu Gdańskiego mogą starać się o wyjazd na
semestr do University of Wisconsin-Parkside https://www.uwp.edu/. Oferta wymiany
obejmuje zwolnienie z czesnego. Student sam musi pokryć koszt przelotu, utrzymania, a także
opłat związanych z procedurą wizową, posiadać środki na zakup książek, ubezpieczenia etc.
Do podania o wyjazd (forma listu motywacyjnego) należy dołączyć zaświadczenie o poziomie
znajomości j. angielskiego oraz o średniej ocen uzyskanej w poprzednim semestrze. Podania
należy składać w Sekretariacie Instytutu Historii (pok. 2.54) w kopercie zaadresowanej do: dr
Anny Mazurkiewicz.

• Health Insurance:
*UW-Parkside health insurance plan is required for all international students. For some exchange programs, the insurance may be
waived ONLY after a thorough review of an alternative plan from the student’s home country. This alternative health plan will be
approved by the Offices of the Dean of Students & the International Student Office.
To be approved for a waiver, a complete English copy of the alternative insurance plan should be submitted and the WAIVER
FORM completed and sent to the International Student office. The alternative health insurance policy should be
a maximum of $500,000 in coverage for emergency care, hospitalization, evacuation and repatriation.

Applications should be sent by June 1st to
Admissions Dept. http://www.uwp.edu/apply/admissions/international/
Once the above documents are received by the Admissions Dept., students will be sent an acceptance letter &
an I-20 document. They will then pay an on-line SEVIS fee ($200) & schedule an appointment to meet with a
U.S. Visa officer to receive an F-1 visa to enter the country. Once they receive a visa, they can book a flight to
the U.S. and send our office (International Student Services) information about their flight so that we can
arrange airport pickup
