RESIDENTIAL ASSISTANCE PROGRAM (RAP) GUIDELINES
GOAL
The Residential Assistance Program (RAP) is intended to provide financial rental assistance to
indigent / low income adult clients receiving Community Psychiatric Supportive Treatment
(CPST) services from a contract agency of the ADAMHS Board so a Client can live in an Adult
Care Facility (ACF) in the community.
RAP is targeted for indigent / low income adult clients that are discharge ready from an
institutional environment such as a hospital, nursing home, jail, Residential Care Facility,
Respite, or Crisis Stabilization Unit.
CRITERIA FOR ADULTS
1. The Client must currently be receiving CPST services from ADAMHS Board contract
Agency.
2. The Client must currently be residing in an institutional environment and need to
transition to an ACF.
3. RAP Application must be approved prior to release from the institution. If Client leaves
the institution before RAP is approved the Client shall be responsible for the cost of the
ACF.
4. Client must meet the level of care provided by the ACF and demonstrate that he/she will
benefit from the services provided by the ACF.
Utilization of these funds will be limited after all available sources such as professional samples,
indigent care programs, Central Pharmacy, Department of Health and Human Services Disability
Assistance Program (if available) or any available agency funds earmarked for this purpose have
been exhausted. Assistance is limited to psychotropic medications and clinical appropriate
treatments / monitoring on a time limited basis not to exceed three (3) consecutive months while
other resources are identified and accessed. This shall be determined on a case-by-case basis.
PROCESS
1. Client and CPST complete RAP Application and then submit to CPST Supervisor for
review and signature consistent with the treatment team recommendation.
2. CPST submits the completed original signed Application to the ADAMHS Board.
3. Only ACFs that have a contract with the ADAMHS Board can be selected.
4. If the Client receives Social Security benefits, the Residential State Supplement
(RSS) Program must be applied for and verification of the RSS application must be
submitted with the RAP Application.
5. Resources Specialists at the ADAMHSB review the Application. Other than the
Application form, additional information required include, but are not limited to,
verification of Social Security income, copies of latest 3 bank statements or Notarized
Statement of No Bank Account (explained on page two to the Application), and any other
financial paperwork associated with the Client.
6. Incomplete applications will not be processed and will be returned to the sender.
7. Once all required paperwork is submitted, notification of approval or denial for RAP will
be faxed by the Resource Specialist to the CPST, CPST Supervisor, Housing Liaison,
ACF Operator, Payee, and ADAMHS Board finance department within five (5) working
days.
FINANCIAL ARRANGEMENTS
1. CPST Worker and Payee are required to update ADAMHSB Board immediately
regarding any change of income, including but not limited to any lump sum payment
from Social Security. Failure to respond to the request for information shall result in the
Client’s termination from the program.
2. Client must sign the Loan Payback Agreement on page three of the Application. In the
strictest sense, RAP is a loan program that funds services at ACF’s. RAP funding is not
income for the Client.
3. Assuming approval, the ADAMHS Board will release funds directly to the entities
identified on the Application within 30 calendar days of approval.
4. Annual financial audit shall be conducted on all RAP recipients.
5. If Client remains in the ACF after termination from RAP, consumer is fully responsible
for payments to the ACF.
