APPLICATION FOR A DEPENDANT'S PASS TO ADOPT A FOREIGN CHILD
Dear Applicant/s

Please note that you are required to bring the Original copies of documents for
verification. Otherwise please submit Certified True Copies.

Incomplete submissions will not be accepted.

Your application will take about two weeks to be processed from the date of
submission of all required documents.

Please provide an official translation if any of the above documents is not written
in English.

ADOPTIVE PARENT(S)
For Singapore Citizens and Singapore Permanent Residents – Identity Cards
For Foreign Citizens – Passport and Employment Pass / Dependant Pass
which appears as a stamp in the passport
Letter(s) of employment stating date of commencement, designation and
salary per month (please submit latest pay slip if the letter of employment was
issued more than 1 month ago) or copy of Business Registration Certificate, if
self-employed
CPF statement(s) showing monthly CPF contribution for the last 12 months
Income Tax Notice of Assessment for the last 3 years
A copy of the Home Study Report
For foreign prospective adopters - Letter stating the intended citizenship of
the foreign child or an approved Letter of Assurance from the Embassy/High
Commission on Entry Visa for the child into the country of the prospective
adoptive parent(s)
An itemized breakdown of the cost incurred with proof of receipts and
statutory declarations if you have made payment to any person or agency for
the transfer of the child either directly or indirectly. Eg. Receipts from law firm,
hospitals, or individuals (with details such as NRIC, Name, contact details etc)
whose services you paid for directly or indirectly.

DOCUMENTS (PHOTOCOPIES) TO BE PRODUCED FOR THE
FOREIGN CHILD
A copy of child’s Birth Certificate with a separate note indicating where the
child was obtained i.e. through commercial agencies (to state agency’s name)
or otherwise
A copy of the child’s Student Pass or Long Term Pass (If any)
A copy of the child’s passport (all pages of the passport required)
The consent for adoption from the child’s biological parents, stating the full
particulars of the child’s biological parents and the prospective adoptive
parent(s).
If either of the biological parents is under 21 years old, his/her parents’ written
consent must also be obtained. If the identity of one of the biological parents
is unknown, the known biological parent must provide an affidavit stating that
he or she is not aware of the identity or whereabouts of the other parent and
to give brief reasons why. The consent of all relevant persons must be given
in the Court’s prescribed format and be signed before a commissioner for
oaths/notary public.
One recent passport-sized colour photograph of child. Please write child’s
name at the back of the photograph and do not staple the photograph. The
photograph must be:
o 35 mm wide by 45 mm high without border and taken within the last three
months;
o taken full face without headgear, unless the applicant habitually wears a
headgear in accordance with religious or racial customs. The headgear
must not hide the applicant’s facial features; and
o the facial image must be 25 mm and 35 mm from chin to crown
