Updated: 4/20/15 FY15 Staff Performance Assessment Process and Timeline Supervisors must meet with each employee to complete the employee’s annual performance assessment. These meetings should be scheduled at least two weeks in advance. Also, at least two weeks prior to the meeting, supervisors must give each employee a copy of the employee’s FY15 documented performance goals, job profile, and a self-assessment and FY16 goals form to complete. In preparation for the performance assessment meeting, the supervisor should consider and assess the employee’s performance against the employee’s documented performance goals and job profile, considering the employee’s completed self-assessment and any file notes or other records the supervisor has maintained documenting the employee’s performance throughout the year. Type in your UST Username and Password and then click on Login. This will take you to the Welcome page. Click on Employee Services. On the Employee Services page, click on HR Job Profiles. On the HR Job Profiles page enter the employee’s UST ID or position number of the job and click on View job profile. On the HR Job Profiles page select the profile to view or print. Action On or Before The employee completes the Self-Assessment and Goals form and gives it to the supervisor at an agreed upon date. June 15, 2015 The supervisor conducts the performance assessment discussion with the employee. June 30, 2015 The supervisor completes the Performance Assessment form after the performance discussion and gives it to the employee for review. July 15, 2015 The employee may add comments to the Performance Assessment form, if desired, and must sign and return the form to the supervisor. July 22, 2015 The supervisor signs the form, and submits the form to his/her manager to sign. July 24, 2015 The supervisor must submit the signed original completed Performance Assessment July 29, 2015 form to HR and provide a copy to the employee. Completed original forms will be placed in the employee’s personnel file. Employees and supervisors who do not submit performance assessments will be ineligible for salary increases. Page 1 of 1
