Open Your Web Browser
Make Sure you are connected to Internet

Users from BHOPAL can directly write bhopal.pcpndt.in

First time users
Enter Your Center Registration Number as Login Name.
Enter your Registration Number as your Password
Your Registration Number must be a 8-10 digit number separated by “/”
For Ex : 035/03/2002
Whatever your registration number is, use it as it is with Zero and Slash
( if present).

On First Time Login, System will ask you to change Password, Here you can
enter password of your choice which you can remember.

Once your password is Changed, you will see this screen

How To Fill Form-F
Step – 1
Click Form-F from Menu at Top
You will see a List of FormF Filled for your center,it will be Blank for First Time
Users.
By default system will show only the Final Submitted Forms .

Click “Add FormF” Button
This will Open a New FormF To Be Filled.
Your center details will come PreFilled
Note :
MCTS ID : It is Mother and Child Tracking System ID given by Govt, in case you
don’t have it, you can skip it.
Once you select No. Of Boy and Girl, you will get option to enter their age.
At Bottom there is a Checkbox Skip Field Validations : If you select it System
will NOT ask for any Compulsory fields and you will be allowed to Press NEXT.
On each Page this Option is given to ByPass Compulsory Field Validations

On Last Page
You will find Checkbox at Bottom :
Check,If You Confirm All The Entries Of Patient Is Filled. But Be Sure Once You
Mark it Checked ,You will NOT be Able to Edit Patient Record.
On Clicking Checkbox and Submit Button, Form gets Submitted to CMHO
Office.
Only after this you will get Acknowledgement Mail on your Registered mailed.
On Clicking Cancel ,Form will NOT get submitted and will be Deleted
