Elite Driving Academy LLC 720-388-0709 The following party __________________________________________, parent of __________________________________ agrees to the following terms set forth by Elite Driving Academy. Brighton Session: Oct 3,4,10,11 Sat 7:30-4:30 Sun 10-5pm Class location Brighton Rec Center 555 N 11 th Ave Select an option from below. (Put parent initials by ONLY one) __________$420 includes class, permit test and six hours behind the wheel training once permit is issued. __________$160 This option includes ONLY the class and permit test. No behind the wheel is provided. Any information in this contract pertaining to driving will NOT apply to this option. The written permit test is given to any student within one month of turning 15 or older and the certificate for test expires after 30 days. Students are allowed to test up to two times with a driving school, and if they do not pass, they are required to retest at the DMV. Any student missing more than 10 on the first attempt is not allowed to retest a second time on the same day and will need to make arrangements to retest at a DMV. Not all students will be of age to take the written test during the session and are welcome to attend another session to do so, however it will not be feasible to set up individual appointments to test, nor is there any guarantee that another session will accommodate to student’s birthdates. All written permit tests are administered at the DMV free of charge, and students are encouraged to go there if necessary. The driving portion of this contract is valid starting from the time the learners permit is issued until one year thereafter. You have exactly 6 months from the time your child completes/ passes the 30 hour classroom portion to have him/her tested for the learner's permit. If you fail to do so, Elite Driving Academy is not responsible, and you are solely responsible for enrolling the student in and paying for another 30 hour driver's education class of your choice. All certificates have expiration dates set forth by the Department of Revenue. By enrolling my student in the driver's education training, the student is in no way guaranteed a passing grade on either the driver's education training class, the written learner's permit test, or the driving skills test. Only after the student has earned his/her learner's permit, that he/she has exactly one year to complete the 6 hour behind-the-wheel training. If the above party fails to complete the 6 hour-behind-the-wheel training within the one year time limit, Elite Driving Academy will not issue a refund.s I understand that my student must abide by all rules set forth by Elite Driving Academy and if a situation arises where the student is in danger due to his or her failure to follow rules, the student will be dismissed from the program without a refund. I understand that Elite Driving Academy will only sign off on driving time when it is actually completed. I understand that any type of cheating will not be tolerated and is reason for dismissal from the program without refund. I understand that there will be a $75 fee if my student fails to show up for a scheduled driving session without giving at least 24 hour’s notice via phone call. I understand that it is my responsibility to keep track of my student's logs and paperwork issued to me by the driving academy. I also understand that the DMV only accepts original copies of any document. There is a $10 fee for any reissuing of paperwork or certificates that are lost by the student or parent. There is a $30 fee if the written test certificate expires and a student needs a retake. I understand that my student must return all classroom materials including books to the instructor in the condition they were checked out. Certificates of completion will only be issued when all materials are turned in or fines are paid. The fee for soft cover text book is $40, and the hard cover is $60. Students are not allowed to have a cell phone or any other electrical device on when training behind the wheel. If my student requests to use the restroom or take a break during behind the wheel training he or she may do so when the instructor feels it is safe to pull the vehicle over at the appropriate location. I also know that if I wish for my student to have a snack during the behind the wheel training, I, the parent, must provide either the money or the snack, and it is up to the student to verbally request a stop. Elite Driving Academy reserves the right to impose a fuel surplus charge of $15 if gasoline prices meet 5.00 a gallon. If gas exceeds $5.50 per gallon, Elite Driving Academy reserves the right to post phone driver’s training until gas prices are $5.50 or lower. I give my student permission to leave campus during approved lunch breaks and do not hold Elite Driving Academy responsible for any incidents that may occur off of school property. Guardian I will be taking part in arranging pick up and drop off times and locations for my child and there may be times when I, the parent, am running late or a parent may not be home. I agree that my child is responsible enough to be home by him/herself or at an agreed upon location at an agreed upon time. If I am unable to retrieve my child, it is my responsibility to notify the instructor to make other arrangements for drop off or pick up. My student may only drive if he or she has a learner's permit or a valid Colorado Driver's license on his/her body. If a student shows up for behind the wheel training without proper documentation, it is against the law for him or her to drive. A cancellation fee of $75 may be imposed. There may be times where other students/occupants are in the driver's ed. car as either passengers or drivers, and I give consent. This agreement constitutes the entire contract between the school and the student, and any verbal assurances or promises not contained herein are not binding on either the school or the student. Under this agreement an instructor may not provide behind-the-wheel training to more than two individual students per session. Seat belts will be worn while the car is in motion or on the road, and it is not authorized, encouraged or safe for students to unbuckle at any time. Any traffic violations are the responsibility of the individual driving the car at the time of the infraction. Instructor's cell phone will not be answered during behind the wheel training and but I can leave a voice message. ****I know I must take the 30 hour class avadavat, original birth certificate, social security number, proof of residency and a guardian must accompany the student to the DMV to get the permit.
