agrees to the following terms set forth by
Elite Driving Academy for behind the wheel driver's training.
I understand that the fee of $300 goes towards 6 hours of behind the wheel training.

I understand that if I wish to have Elite Driving Academy test my student for the driving test,
there is an additional fee of $40 and that test. All certificates have expiration dates set forth by
the Department of Revenue. I know I must read them.


There is a $25 fee for returned or bounced checks and customer responsible for all collection
fees and interest if any.
I understand that by enrolling my student in the driver's training, the student is in no way

guaranteed a passing grade on the driving skills test.
I understand that my student must abide by all rules set forth by Elite Driving Academy and if a
situation arises where the student is in danger due to his or her failure to follow rules, the
student will be dismissed from the program without a refund.


I understand that Elite Driving Academy will only sign off on driving time when it is actually
completed.
I understand that there will be a $75 fee if my student fails to show up for a scheduled driving
session without giving at least 4 hour’s notice via phone call. After three incidents of a student
failing to notify the driving academy of a cancellation, Elite Driving Academy may choose to

drop the student from the program without issuing a refund.
I understand that it is my responsibility to keep track of my student's logs and paperwork issued
to me by the driving academy. I also understand that the DMV only accepts original copies of
any document. There is a $10 fee for any reissuing of paperwork or certificates that are lost by
the student or parent

I understand that students are not allowed to have a cell phone or any other electrical device on
when training behind the wheel. There is to be no eating or drinking in the car.

I understand that there may be a camera and or recording device placed in the training car for
instructor training purposes and any footage or output is the sole property of Elite Driving

Academy.
I understand that if my student requests to use the restroom or take a break during behind the
wheel training he or she may do so when the instructor feels it is safe to pull the vehicle over at
the appropriate location. I also know that if I wish for my student to have a snack during the
behind the wheel training, I, the parent, must provide either the money or the snack, and it is up

to the student to verbally request a stop.
I understand that I am solely responsible for my child until he/she is picked up by an instructor

from Elite Driving Academy. I know I may wait for the instructor to show up before I leave my
child, and I assume full responsibility for my child if I leave him or her unattended.
I acknowledge that I will be taking part in arranging pick up and drop off times and locations for
my child and there may be times when I, the parent, am running late or a parent may not be
home. I agree that my child is responsible enough to be home by him/herself or at an agreed
upon location at an agreed upon time. If I am unable to retrieve my child, it is my responsibility
to notify the instructor to make other arrangements for drop off or pick up.

My student may only drive if he or she has a learner's permit or a valid Colorado Driver's license
on his/her body. If a student shows up for behind the wheel training without proper
documentation, it is against the law for him or her to drive. A cancellation fee of $75 may be
imposed.

I understand that there may be times where other students/occupants are in the driver's ed. car
as either passengers or drivers.


Seat belts will be worn while the car is in motion or on the road, and it is not acceptable,
encouraged or safe for students to unbuckle at any time.
I understand that the instructor's cell phone will not be answered during behind the wheel

training and I must leave a voice message if I need to get in touch with the instructor.
Traffic violations and any fees associated with one incurred is the driver’s responsibility with the
exception of parking tickets.


This agreement constitutes the entire contract between the school and the student,
and any verbal assurances or promises not contained herein are not binding on
either the school or the student.
Under this agreement an instructor may not provide behind-the-wheel training to
more than two individual students per session.
