BROOKFIELD SCHOOL PTA END OF THE YEAR POOL PARTY
HOLD HARMLESS AGREEMENT FOR BROOKFIELD SCHOOL AND BROOKFIELD PTA
The Brookfield School PTA End of the Year Party will take place at Wild Island on Friday, June 12,
2015. I hereby consent to my child(ren) taking party in this trip and expressly relieve, indemnify, and
hold harmless Brookfield School and Brookfield School PTA in Reno, Nevada and all respective
directors, employees, officers and volunteers thereof (collectively referred to as “Brookfield”) from and
against any and all liability or claims arising from injury or damage to person, property, or both,
caused by or resulting from my child(ren)’s acts, omissions or conduct while on this trip.
I also release and relieve Brookfield from any and all liability of claims arising from injury or damages
suffered or incurred by my child(ren) as a result of the acts, omissions, or conduct of any person,
other than the negligence of Brookfield. It is further understood that I shall have the responsibility of
advising my child(ren) of the risks which are known, or should be known, on this trip. I further agree to
assume the responsibility of seeing that my child(ren) cooperates and conforms to the fullest extent
with Brookfield and any instructions from Brookfield pertaining to this trip, as well as all park
regulations/rules set forth by Wild Island for their water park guests, particularly in regards to adult
supervision of minor park attendees.

______OPTION 1. I will be transporting my child(ren) to the water park event at Wild Island. I
understand I am responsible for transporting my child(ren) to and from the water park and for
providing appropriate supervision for my child(ren) at all times during their time at the water park, in
accordance with all Wild Island water park facility rules and regulations, as mandated with water park
admittance.
______OPTION 2. I cannot attend the water park event at Wild Island. I have asked
____________________ (Authorized Adult/Parent’s Name) and he/she has agreed to transport and
supervise by child(ren) at all times during their time at the water park, in accordance with all Wild
Island water park facility rules and regulations, as mandated with water park admittance.

The End of the Year PTA Pool Party is Wednesday, June 11th. Lunch will be available at Main Campus at noon.
The Elks Club (next door to TLC) is opening their pool for a splashy good time at 1:00 – 3:30 pm. All students who
wish to attend will need a signed waiver from a parent and transportation arrangements both to and from the Elks
Lodge. Please click here for the waiver. Don't forget your bathing suit, sunscreen and towels!
