It is for information of all concerned that the Original Pass Certificate, Migration
Certificate, Marks Memorandum of fail candidates and schoot copy of tabulation register
will be made available in the disbursal centres as detailed below. The Head of the
lnstitutions or their authorized signatory shall collect the above records after verification on
spot. They have to produce their school identity card at the counter for receipt of the
certificates. The missing of any certificate must be reported at the counter on the day of
receipt of the certificates.
The corrections if required in original pass certificate(s) of any candidate(s) the
concerned Head of the lnstitution should approach their concerned zonal office with
original documents such as Admission register and cancelled TC for verification on or
before 1010612015 with deposit of requisite fees. The concerned Deputy Seceretary
should send the consolidated list of correction of Original Pass Certificates to the Head
Office, Cuttack by 1710612015. No complaints regarding corrections will be accepted
thereafter.

1. The District Education Officers concerned for information and necessary
action.
They are requested to inform the schools to collect their certificates etc. from the
selected centres as per the programme detailed in the above notification by
producing their school identity card.
2. The Deputy Secretaries of all zonal offices of the Board for information
and
necessary action. They are requested to collect the above materials by ananging
the vehicles as detailed below. The Central/ Bhubaneswar/ Balasore Zone shall
collect their above materials on 2110512015. They are also requested to depute
their staff to respective disbursal centres for distribution of above materials by 10
A.M on 2210512015 as per scheduled programme.
