Preschool children must be 3 or 4 on or before 9/1/2015.
Appointments are recommended, but walk-ins will be accepted.
Receive a free backpack filled with activities and
tools to increase fine motor skills at home.
Programs are FREE or tuition-based for families that qualify.

What to Bring to Registration (this information is required):

Your child’s up-to-date IMMUNIZATION RECORD

Your child’s CERTIFIED BIRTH CERTIFICATE (the certificate from the court house)

PROOF OF INCOME for the last 12 months – one or more of the following:

-2014 tax forms (W-2 Form); Three most recent check stubs OR
unemployment statement; DHS Statement (FIP, SSI, etc.) or Day Care Assistance form

PROOF OF RESIDENCY (something with your current name and address on it:
driver’s license, rent receipt, utility bills or property tax bill)

