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ontract No. 201521051 Structure Numbers Subset Sheets: 2 of 4 Sheet Number 20 Spill Prevention, Control, and Countermeasure (SPCC) Plan \\diagis05\DIA-Projects\Landside\Projects\2015\010_BridgeRehabPHI(SafetyImprove)\08.CAD_Drawings_Directory\BRREPH1(SAFETY)_SMDT02.dwg The Contractor shall prevent oil or other hazardous substances (as defined in federal and state regulations) from entering the ground, drainage or local bodies of water, and shall provide containment, diversionary structures, or equipment to prevent discharged oil from reaching a watercourse and take immediate action to contain and clean up any spill of oily substances, petroleum products, or hazardous substances. The Contractor shall provide one or more of the following preventive systems at each petroleum storage site: Dikes, berms, or retaining walls capable of containing at least 100% of the volume of the largest single tank and equipped with sufficient freeboard to contain precipitation events. The secondary containment must be "sufficiently impermeable" to prevent a release to the environment. Culverting, curbing, guttering or other similar structures capable of containing at least 100% of the volume of the largest single tank. The provision of such preventive measures shall be subject to acceptance by the DIA Project Manager prior to tank installation and shall follow the SPCC regulations (40 CFR Part 112). Prior to bringing any containers of 55-gallon or above capacity onto DIA property for storage of oil, fuel, or other petroleum substances, the Contractor may be required to prepare an SPCC Plan that conforms to 40 CFR Part 112. The plan must include either a certification from a Professional Engineer or self-certification (if applicable), as well as management approval from the legally responsible Contractor representative. Reporting Oil Spills The Contractor is responsible for all spills that may result from its activities. For ANY suspected or confirmed release or spill of oil, fuel, solid waste, hazardous waste, unknown materials, lavatory waste, or miscellaneous chemicals, etc. that occurs as the result of the Contractor's activities on DIA property, the Contractor is required to take immediate action to mitigate the release or spill and report it to the DIA Project Manager and to the DIA Communication Center at (303) 342-4200. The Contractor is responsible for notifying the appropriate regulatory agency(ies) in the event suspected and/or confirmed releases are identified, in accordance with regulatory requirements.
