Attention Residents of
Montgomery County
Adults between the ages of 18 and 64 must have
health coverage, or pay a tax penalty. If you paid a
2014 tax penalty, received a reduction in your 2014
tax refund, and do not have health coverage, there
is still time to avoid a larger penalty for the 2015
tax year. ENROLL TODAY to reduce 2015 tax year
penalties! The sooner you enroll, the less you pay.

A government issued ID with birth date; a social security
number; if employed, a pay stub or w-2 wage and tax
statement; policy numbers from other health insurances.
You must confirm that you were taxed for not having health
insurance in 2014.
