2 MONTHS (60 days) PRIOR TO THE RATING,
All Fees associated with the Rating must me Turned in with this completed Form.
If this is not done 2 months (60 days) prior to the rating, you will not rate.
It is understood that the member rating must have all flow sheets completed, and receive
signatures below. It is the responsibility of the member to have form completed prior to the
deadline date.
