Who Should Attend – Given the interdisciplinary nature of wound care, topics covered in this Program will appeal to a wide range of primary
care providers, including (but not limited to) those in the South West LHIN that have been designated by their organization as a SWRWCP ‘Wound
Care Champion’, physicians, nurses, and therapists (occupational therapists, physiotherapists, etc.). Managers and practice change leaders of such
primary care providers are STRONGLY encouraged to attend, as based on the contents of the Program the need for organizational practice change
will be realized. This Program is not appropriate for those with advanced knowledge in wound care, i.e. those with an ET certificate (or
equivalent), IIWCC certification, or a MClSc degree.
Event Details - In collaboration with the CAWC, the SWRWCP is pleased to offer a wound care educational Program that offers professionals
with the opportunity to broaden their knowledge base and to meet with others who share their concern in improving quality wound care. The

Program consists of five on-line modules (to be completed BEFORE attending the Program April 22 ) and a two day interactive case-based
workshop, led by an interprofessional faculty, which focuses on the prevention and management of various wound types, i.e. pressure ulcers,
lower-limb ulcers, diabetic foot ulcers and acute wounds, via the utilization of local resources and critical thinking skills. This is NOT a dressing
workshop.

positions)  this includes access to the five online modules,
admission to the two-day workshop, daily lunch and refreshments at
the workshop, Program materials, access to CAWC education extras

Notice of registration withdraw must be received via e-mail
([email protected]) by April 6, 2015 to receive a full
refund. No refunds will be issued to those who withdraw after April
6, 2015 – without exception. Registered participants who are unable
to attend are welcome to send a colleague in their place (please
notify [email protected] of your replacement’s name
and email address should this occur).

Upon receipts of your completed registration form, Michele will forward your name and e-mail address to the
CAWC who will send your information to the Ontario Hospital Association who will e-mail you a passcode and
instructions on how to access the five on-line modules which MUST be completed before attending the
educational Program April 22-23rd.

Registration Process for Non-Funded Positions
1. Complete the registration form in its entirety
2. Fax the completed registration form AND a photocopy of your $525.00 cheque (made out to the South West
CCAC) to 519-527-1255 (attention Michele Dalton) or email the copies to Michele at [email protected]
3. Mail the original version of your registration form AND your original cheque to:

4. Once Michele receives the faxed or emailed copies of BOTH your registration form and cheque, she will forward
