Complex Cardiovascular Therapeutics
Remarkable progress in technology brought us the chance to tackle more and more complex
coronary lesions. Chronic Total Occlusion (CTO) is one of these complex coronary lesions that
we have been working on to conquer. Recent Japanese multicenter CTO registry data has
shown a gradually improving success rate and a very high success rate was achieved by some
expert operators. However, as the retrograde approach becomes an established method and
new device technology prevails widely, CTO PCI becomes not to be restricted to some expert
operators.
Greetings from the 16th CTO Club
Why do we fail in CTO PCI? The answer is clear. It is because we cannot see the route in the
CTO lesion. If we can see the route like in non-CTO lesions, we can surely trace the route
except anatomically impracticable lesions. Coronary MDCT provides us useful information
about the lesion. Even though using these imaging modalities, you sometimes see the best
expert (most experienced) operator being trapped in difficult situation during the procedure. It
happens when the image route moved away from the correct one. Several methods to lead the
wire to correct route, for example, parallel wire technique, reverse CART, and IVUS guided
reentry, have been applied. However, there seems to be rooms for technical innovation to
establish these methods.
The advent of DES and new anti-platelet therapy has reduced restenosis and reocclusion rates.
Due to this beneficial effect, recanalization of CTO improves LV function and provides better
long term outcome. Recent studies about STEMI with multivessel disease have shown that the
existence of CTO lesion is one of the most important factors of poor clinical outcome. It means
that it is significant to open the CTO vessel in various lesion subsets.
The purpose of CTO Club is to inform you useful and important technical innovations from
various aspects through dynamic live demonstration. CTO is challenging; therefore, worth
doing. Come to Nagoya and share various CTO cases. We are really looking forward to seeing
you at the 16th CTO Club in June, 2015.

Retrograde Summit 2015

Initial results of CTO PCIs in domestic and foreign high volume centers
have improved, with success rates reaching more than 90 percent. The
biggest reason for this high success rate is the introduction of the
retrograde approach.
The retrograde approach was invented in 1995 and was standardized
with the availability recently developed and introduced devices.
Contemporary Reverse CART, which was introduced at last year’s CTO
Club, is a typical example of a technique made possible by this
innovation. In addition, regarding CIN which is considered as a problem
in CTO PCI, it has been reported that contrast medium usage could be
minimized by applying retrograde approach with utilization of IVUS,
mitigating CIN which has traditionally been seen as a problem in CTO
PCI. Furthermore, thanks to advancement in X-ray devices, treatments
with lower doses of radiation even in long-lasting treatments have
become available.
The introduction of J-CTO Score as an indicator of difficulty in CTO PCI,
and the possibility of achieving very high revascularization success
rates even in cases with high J-CTO Score by application of retrograde
approach is reported; however, retrograde approach requires an
existence of collateral channel suitable for intervention, which remains a
clear Achilles’ heel of this approach.
This year’s Retrograde Summit will provide not only the latest data
regarding retrograde approach, but also utility and usage of newly
developed or introduced devices along with a live case.

Fireside Session: Proctorship Live

In this Proctorship Live demonstration, the less experienced physician will act as the first operator, while the expert physician will act as the second
operator. The less experienced, younger physician, will of course have a strong desire to master CTO procedures and therefore will engage directly in
the case. Meanwhile, the expert physician will remain close by to advise throughout the procedure. By having the younger physician engage directly in
the procedure, he or she will be able to acquire the tips, tricks and techniques to complete the case successfully, and be educated on the common
pitfalls unique to young operators. Our goal is to share CTO knowledge, and teach techniques for successfully completing complex CTO cases with
high proficiency and safety.
