A MODERN TEA
In 2013 A Modern Tea went Green!
The focus was on Eco-friendly and green livingboth inside and out. The event was hosted at
the beautiful New Haven Lawn Club in New
Haven where more than 170 guests were treated
to a full day of exciting and empowering health
and wellness workshops and activities.
Key Highlights:
•American Heart Association and the Women’s
Heart Program at Yale-New Haven Hospital conducted a variety of beneficial health screenings. •Dr. Anita M. Kelsey led an informative and interactive presentation that taught guests where they
ranked in terms of BMI and blood pressure, and
how those numbers affect heart health. •Toxic chemicals expert Anne Hulick discussed
how the chemicals found in common household
products could adversely affect your health. K HAIR

•Fitness expert Robert Fulton explained how adding 30 minutes of exercise per day and incorporating healthier eating habits can improve heart
health. •
Michael Harney of Harney and Son’s Fine Teas presented an engaging tea tasting workshop discussing
how properties in teas can help improve heart health. •Wellness practitioners graciously volunteered
their time to provide relaxing massage and Reiki
services to our guests.
•The evening was completed with a fashion showcase hosted by Ali Reed of News Channel 8, clothing from The Dressing Room of Wallingford, hair/
make-up by Captiva of Wallingford, and footware
by The Tori Soudan Collection.

TAKING CARE OF KIDS ONE HEAD AT A TIME
KHAIR Graduation Performing Arts Showcase
This marks the second year that the KHAIR
graduation has included original performances by
our participants. Without fear or reservation, they
stepped out in front of friends, family, their stylists
and clinicians to showcase their art with poise and
confidence.
Each year they continue to impress and pull on our
heart-strings with their talent and bravery. We heard
inspiring reflections on how they’ve gone from low
self-esteem or being bullied, to feeling good about
themselves.
As part of the KHAIR program, teens are paired
with professional hairstylists and barbers who
provide complimentary hair services and personal
appearance coaching. Participants also attend
life skills training workshops in the following
topic areas: self-esteem, social etiquette, nutrition
education, financial literacy, team building, and
dress for success, bolstering their self-esteem.
KHAIR First Salon Fundraiser
On March 23, 2013, Jo Bruno Hair salon in
New Haven hosted the first salon fundraising event
for KHAIR.
It was wonderfully attended and a great success,
raising over $1,500. The Jo Bruno Salon, who has
been supporting the KHAIR program for just over
two seasons, approached us with the idea for a
fundraising event called Curl’s Night Out.
Curl’s Night Out was designed to help women
elevate their self-esteem by proudly wearing their
curly hair while supporting a good cause. The
event coincided with KHAIR’s mission to boost
self-esteem levels in at-risk New Haven area youthone head at a time. The salon received support
from clients, local businesses and staff in hosting a
beautiful, informative, and entertaining evening.

RAISES MORE THAN $288,000 TO DATE
For the fourth year in a row, the golf classic was held at one of the most premier courses in the state Lake of Isles at Foxwoods Resort Casino. More than 120 golfers participated including 23 corporate
teams. We would especially like to thank Great Blue for being this year’s diamond sponsor. The Great
Blue corporate team even took third place overall.
We’re happy to announce that we will once again be returning to Lake of Isles for next
year’s tournament.

New London County’s First
Culinary Program
Griffin Hospital
Partnership
Cooking Matters CT entered into a partnership with
Ledgelight Health District’s program ACHIEVE in
2013. With the help of volunteer chefs and dietitians,
the ACHIEVE New London County Coalition
launched the county’s first culinary program for
middle school students and human service providers.
Griffin Hospital VITALS Committee partnered
with Cooking Matters in May of 2013 to bring
Cooking Matters for Kids to Irving Elementary
School. This was a unique approach, conducting
workshops during the school day and to all 5th
grade Science teaching blocks. The 5th grade
science teacher saw this as a way to teach the
nutrition curriculum, engaging her students in an
interactive and fun way. Her principal agreed and
gave the partnership the go ahead. Cooking Matters
staff quickly got to work organizing and planning
this huge undertaking as each of the classes had
approximately 20 children. By the end of June
2013, each 5th grader had successfully graduated
from Cooking Matters for Kids, and proudly
displayed their graduation certificate; sharing
their favorite recipes. Due to this model working
so well, it has been determined that an
additional school will be added in
the Spring of 2014.
The ACHIEVE Coalition hosted a Cooking Matters
facilitator training for 33 local chefs and dietitians,
and began offering the six-week Cooking Matters
program to diverse populations with several pilot
programs operational throughout New London
County schools and community organizations. The
program was launched at Cutler Middle School
for students enrolled in the Student Wellness and
Activity Team program before making its way to all
New London County middle schools with schoolbased health centers.
A Cooking Matters train-the-trainer program for
human service providers was held October 23,
2013 at the Thames Valley Council for Community
Action Commissary and featured guest volunteer
chefs and dietitians who led hands-on preparation of
full meals, using a combination of fresh produce and
food pantry items.
“Our hope is that by providing this training to
human service workers, they will teach what
they have learned to the individuals and families
that they serve,” said Cindy Barry, Senior
Health Program Coordinator for
Ledgelight Health
District.

BY THE END OF THE SIX-WEEK COURSE, ADULT
AND TEEN GRADUATES REPORTED THAT:
• 61% are eating more fruits and vegetables
• 60% are eating more whole grains
• 77% improved their cooking skills
• 82% are now reading food labels
CHILD GRADUATES ARE:
• 4 4% more confident in making
snacks with fruits and veggies
• 38% more confident in talking to
parents about healthy food choices
GRANT MAKING PROGRAM
Over the past ten years the Foundation
has awarded a total of $224,350 to FQHC’s statewide:
Each health center was awarded $4,000 in 2013.
Charter Oak Health Center, Inc. —
To Your Health! Project
Charter Oak Health Center hired a consultant to
provide branding and materials for community
outreach workers and other employees in building
awareness of the health center. Charter Oak
continues to define the organization’s brand and
establish their individuality in the community
they serve.
Cornell Scott-Hill Health Center – Access 360:
Ensuring Access to Care through Improved
Communications
Cornell Scott-Hill Health Center’s project was
focused on utilizing mobile texting, email, and
direct voice communications to increase awareness
of, and compliance with, healthcare programs and
treatment plans. The grant provided them with the
opportunity to incentivize patients and obtain their
communication preferences.
Fair Haven Community Health Center – Patient
Telephone Access Improvement Project
Fair Haven Community Health Center hired a
consultant which allowed them to evaluate their
phone system. This grant not only funded the
evaluation, but assisted with training and transition
costs. The evaluation of their current phone system
ensured lower wait times and accurate feedback for
their patients.
Generations Family Health Center, Inc. –
Production of Generations’ Video for Outreach
and Education Purposes
Generations Family Health Center further expanded
its ability to conduct outreach and health education
by creating educational videos regarding a variety
of healthcare topics and services offered through the
health center. These videos were utilized at outreach
events, on their website, and in waiting areas.
This grant assisted in writing and producing these
educationally branded videos for both their current
and potential patients.
Optimus Health Care, Inc – Loving Support
Through Peer Counseling
Optimus Health Care’s grant afforded the
training to ‘identified’ mothers in the evidencebased, “Loving Support Through Peer
Counseling: A Journey Together” curriculum.
Peer counselors assisted new mothers
who required assistance and support with
breastfeeding. The funds went toward training
and technical assistance programs that enhanced
initiation and duration rates.
Southwest Community Health Center –
Improvement of UDS Measures in Internal
Medicine & Pediatrics
Southwest Community Health Center utilized
the grant to continue their plan that proved to be
an effective strategy in achieving better clinical
outcomes for the population they serve. The
program, presented by a committed clinical team,
provided eight in-service programs; four on
diabetes and four on hypertension.
Staywell Health Center – Prenatal Program/
Nurturing Families Network
Staywell Health Center continued to assist in
funding prenatal ultrasound testing and lab work for
uninsured and low income pregnant patients who
were unable to afford the out-of-pocket expense
for this vital testing. Additionally, this grant funded
their Nurturing Families Network (NFN) program,
offering an educational family field trip.
