Here is your chance to sign up for 6th Annual Cleveland Asian Festival’s Trivia Challenge With
Jeopardy! Champion Arthur Chu!
Be one of the many to compete with Jeopardy! Champion Arthur Chu and have the chance to
become one of the winning team members of Cleveland Asian Festival’s Trivia Challenge With
Jeopardy Champion Arthur Chu Contest!
You must be at least 15 years of age to enter and have a valid state-issued I.D. ready at the
contest.
All you have to do is fill out this Entry Form and submit it by 1pm on Sunday, May 17, 2015, at the CAF Info Booth. All registrants are
subject to an audition and are not guaranteed a spot at the contest, as space is limited. If you are selected, you will be notified
before the contest. The contest starts at 3:15 pm on Sunday May 17, 2015 on the Lantern Stage.
RULES:

Must be 15 years of age or older to enter. Under 18 years of age, Contestant’s parent/ legal guardian must sign along with the Contestant.
Entry is limited to the first 8 qualified applicants..
Contestants will form teams to compete against Arthur Chu on answering trivia questions. The team with the most points will win. The decisions of
the contest judge(s) will be final and not subject to appeal.
CAF reserves the right to reject or disqualify an applicant.

CAF’s Trivia Challenge With Jeopardy! Champion Arthur Chu Contest, I agree to save harmless and indemnify the Organizer, its volunteers and any Sponsors from any
claims, actions, damages, expenses, costs arising out of or related to the Contest. I also consent to the Cleveland Asian Festival retaining all rights associated with any
and all visual and audio recording of the contestants.

Eligibility:
Contest is open to all individuals who are at least 15 years old. Applicants under 18 years old must have their parent or legal
guardian sign the entry form along with the applicant.
To Enter:
Entry is Free. Space is limited. Read carefully the official contest Rules and Entry Form. The CAF’s Trivia Challenge With
Jeopardy! Champion Arthur Chu Contest including the eligibility requirements and waiver of liability provisions. Complete
and sign the Trivia Challenge With Jeopardy! Champion Arthur Chu Contest Form, you can present it on the festival or mail
it back to us at Cleveland Asian Festival, c/o St. Clair Superior Development, 4205 St. Clair Avenue, Cleveland, Ohio 44103.
All entry forms become the property of Cleveland Asian Festival.
The Rules:
Must be 15 years of age or older to enter. Under 18 years of age, Contestant’s parent/ legal guardian must sign along with
the Contestant.
Entry is limited to the first 8 applicants
Contestants will form 2 teams to compete with Arthur Chu on answering trivia questions in order to win most of the points.
The host asks a series of Trivia questions. The quickest of the three teams to signal has an opportunity to answer the
question. If the first respondent fails to answer correctly, the remaining two teams have an opportunity to answer.
Each of the three teams will have a "signal" system to determine who is quickest to respond. The respondent has 5 seconds
to answer after giving the signal or they will be assigned an incorrect answer – and lose point(s)
Ten Minute First round; each question is worth 1 point.
Five Minute Second round ; each question is worth 2 points.
Each correct answer gets 1 or 2 points, each incorrect answer will lose the same number of points.
The team with the highest number of points after two rounds wins. If there is a tie after 2 rounds, there will be a tie-breaker
round with 5 questions will be played among the teams. Each question is worth 3 points.
CAF reserves the right to reject or disqualify an applicant.
Prize Awards:
Cleveland Asian festival T-shirt will be awarded to each of the winning team members. All contestants consent to the use of
their names, images, video/photography, sound bites and likeness by the Cleveland Asian Festival for any and all purpose.
All Registrants must read and sign this waiver before entering:
All contestants agree to abide by any decision of the contests officials and any and all of the contest rules.
All contestants acknowledge that they assume all risks associated with participating in this event including, and all risks of
personal injury, illness and possible lost of life, and risks of damage to or lost of personal property. Contestant, and anyone
entitled to act on the contestant’s behalf, agrees to waive and release Cleveland Asian Festival and all affiliated persons,
entities, volunteers, and groups, their vendors, their advertising, promotion and public relations agencies, co-sponsoring
companies and their affiliates and agencies, from any and all claims and liabilities of any kind arising out of his/her
participation in this event.
Contestant consents to the use of their name, images, photograph, audio and video recording by the Cleveland Asian
Festival for any and all purposes.
Final Authority:
Cleveland Asian Festival has the final authority regarding all contest-related matters.
