ACKNOWLEDGMENT OF RECEIPT
This undersigned acknowledges that Addendum No. 1 for the REQUEST FOR
STATEMENTS OF QUALIFICATIONS FOR UPDATE OF THE PAVEMENT MANAGEMENT
PROGRAM FOR PORT COLUMBUS INTERNATIONAL, RICKENBACKER INTERNATIONAL,
AND BOLTON FIELD AIRPORTS has been received by the undersigned and will be
incorporated in all copies of said Plans and Specifications in the possession of the
undersigned.
It is understood that all proposals submitted in response to REQUEST FOR
STATEMENTS OF QUALIFICATIONS FOR UPDATE OF THE PAVEMENT MANAGEMENT
PROGRAM FOR PORT COLUMBUS INTERNATIONAL, RICKENBACKER INTERNATIONAL,
AND BOLTON FIELD AIRPORTS will be presumed to be based upon full knowledge of
the contents of Addendum No. 1.

This Addendum shall be considered part of the REQUEST FOR STATEMENTS OF
QUALIFICATIONS FOR UPDATE OF THE PAVEMENT MANAGEMENT PROGRAM FOR PORT
COLUMBUS INTERNATIONAL, RICKENBACKER INTERNATIONAL, AND BOLTON FIELD
AIRPORTS and is intended to correct, change, and/or add to the documents as
described below:
SUBMITTAL INSTRUCTIONS
One (1) electronic (pdf only ON CD ONLY), three (3) hard copies, and one (1) Original submittal
shall be provided. Responses to this solicitation should be sealed, and delivered to:

SOLICITATION FOR “UPDATE OF THE PAVEMENT MANAGEMENT PROGRAM”
DUE: “April 30, 2015 by 12:00 noon 2:00 p.m. Eastern Time”
In the event the response is hand delivered, it must be submitted to the Administrative Office
Receptionist so that it is time-stamped upon receipt. The Authority is not responsible for responses
that are not received by the Receptionist in the Administrative Office.
