Please read the following instructions and the application form carefully.
The New Australian passport application contains 18 sections, some of them are relevant for
you, some will not be, depending on your situation.
Section 2: Name, place and date of birth
This information must be exactly the same as on your Australian cardinal document (Birth
Certificate or Citizenship Certificate). The spelling must be the same as on the cardinal
document, no non-English characters are allowed in section 2.
Section 3: Confirming your Citizenship
To prove Australian citizenship the following must be supplied:
1) A Full Australian Birth Certificate (Commemorative birth certificates or extracts of birth
certificates where parents’ full names are not listed cannot be accepted)
OR 2) An Australian Citizenship Certificate and
the birth certificate of your country of birth in English or a valid non-Australian passport.
If you are born in Australia after 20/08/1986 you must provide one of the documents listed in
section 3a. In most cases this will be the most recently issued passport if issued after 01/01/2000
and valid for more than 2 years. If you cannot provide your most recently issued Australian
passport (because it has been reported Lost/Stolen or seriously damaged), we must see evidence
that at least one parent was an Australian national or permanent resident at the time of the birth
of the applicant. This can be one parent’s original Australian birth Certificate or a Permanent
Residency sticker.
Section 4: Name Changes
When your name has changed from what is written on your Australian Birth Certificate or
Australian Citizenship Certificate you must submit evidence of the name change to have your new
name recorded on your passport.
If the name in your most recently issued Australian passport is your current name, and you
have not changed your name, then you can use your expired Australian passport as proof of your
name – even if it differs from what is on your cardinal document. Please complete section 4 in the
Passport application with your passport details.
If your name has changed more than once since the issue of your Australian Birth Certificate or
Australian Citizenship Certificate you must show all documentation, from the relevant authorities,
to prove each name change - eg marriage certificate, decree nisi, deed poll and name change
certificate from the relevant Registrar of Births, Deaths and Marriages (RBDM).
ALL applicants born in Australia and married overseas must register name changes with the
RBDM of their home state.
Applicants born in Australia and married in Australia must obtain an RBDM marriage
certificate. Marriage certificates or Commemorative Certificates issued by celebrants/church
leaders will not be accepted.

Applicants who are born overseas and have changed their name overseas must present
legalised Change of Name documentation in English. The legalisation process is done by the
Foreign Ministry of the country that issued the relevant Name Change document. If the original
document is not in English, you must supply an authorised translation from a certified translator.

If you changed your surname due to marriage, divorce or death of spouse (incl. de facto and
registered partner) or if you have changed gender, you are eligible for a Gratis passport as long
as there is at least 2 years left on your passport. The expiry date will be the same as in the passport
you are replacing.
You must submit a B11 general declaration stating that you are born overseas, reside overseas and
therefore cannot register your name change in Australia. The B11 General declaration is available
through the Embassy website. Please submit documents or ID cards evidencing that you are
known under your new name. If not possible, you must submit another B11 stating why you
cannot submit this documentation.
Section 5: Security question – your mother’s maiden name
Your mother’s maiden name is recorded on the Australian birth certificate, and was registered at
your first passport application. It does not contain non-English characters.
Section 6: Passport type.
Choose your passport type.
Section 7 a + 7 b: Contact details and Emergency Contact
Must be completed. The emergency contact does not have to be an Australian, just a person you
would like us to contact, in case something happens to you.
Section 8. Address.
Please write your residential address. If you are currently travelling, write your home address. If
you are relocating to another country, but lodging the application here, please write your new
address.
Section 9. Previous passport details.
Please indicate your most recently issued passport number.
Section 10: Lost/Theft of most recently issued passport
Only complete this if your most recently issued passport cannot be presented at interview. Please
mark the date of when the loss happened.

If applying online, filling out this information will lead to the passport being cancelled in the
system. If you recover the passport before the interview, you must present the passport, but you
cannot use it to travel on.
Section 11. The Guarantor.
This category of Identifier does not need to hold an Australian passport. However he or she
must have known you for at least 12 months, not be related to you by birth, marriage or de
facto relationship, contactable during business hours and be:
1. An adult Australian passport holder OR
2. An adult non-Australian passport holder currently working in one of the below
mentioned occupational groups:

Accountants – who are chartered or registered accountants with the relevant
National Accounting body.
►
Bank Managers - other than Managers of Bank Travel Centres.
►
Barristers, Solicitors and Patent Attorneys
►
Chartered Professional Engineers
►
Defence Force Personnel - members currently serving in the Defence Force
with at least five years continuous service.
►
Dentists
►
Judges
►
Marriage Celebrants
►
Police Officers in Charge of Police Stations or of the rank of Sergeant and
above.
►
Public Servants- current full-time employees, who have been employed
continuously for at least five years by their current employer.

Medical Doctors

Teachers - full time who have been teaching for more than five years at schools
or tertiary institutions.

Section 16: Proof of Identity
Please write your Australian driving licence information and present this at the interview. No one
has a Birth Card.
Section 17. Signature of lodging parent or adult applicant
The signature must be inside the box. Full name (all names!) of the applicant and date.
Section 18: Signature of adult applicant or child applicant over 10 years.
Sign again in section 18. Make sure it is inside the box and is signed with dark, rich black ink.

Photo guidelines
Please read the following instructions carefully as we wish to avoid delay in your application
form.
Many photos are rejected because the size of the head is too small or when the photo is poorly
printed and has lines through them. We also reject photos when they have markings and creases
on them.
Don’t attach photos with paper clips, stickers or staples! It ruins the photo
Check your photo - these are the most common mistakes we encounter:
□ Head too small? Use a ruler to ensure size of head match our requirements.
□ Red eye? We cannot accept photos with red eyes.
□ Are there any printing lines? Use a magnifying glass if need be
□ Is it out of focus? Blurry lines around eyes and face?
□ Ensure the ink from the endorsed photo does not rub off on the other photo
You must provide two identical colour passport sized photographs. Use a professional photo
shop to have these done to ensure proper quality lighting, paper and print.
NOTE: Australian requirements are different to local requirements!
Photos must
- Not be more than 6 months old and be free of errors such as red eye, scratches, lines in the
printing or a blurred focus
- Photos must have a neutral expression, with mouth closed and must be printed on excellent
quality print. Light background.
- Size of head on the photos measures at least 3,2 cm and not more than 3,6 cm. The head is
measured from the bottom of your chin to the crown of your head.
Please aim for a head measure of 3,4 cm.
Guarantor endorsement must read:
“this is a true photo of [applicant’s full name],” followed by their signature
The basics
•
35-40mm wide and 45-50mm high
•
good quality colour on high quality paper, with no ink or marks on the image
•
plain, light-coloured background (e.g. cream, pale blue or white)
•
appropriate brightness and contrast and show your skin tones naturally
•
uniform lighting (no shadows across or behind the face), sharp focus and clear.

Head coverings
The photograph must show you without any hat or other head covering.
However, if you wear a head covering for religious reasons we will accept a photograph of you

Glasses
Even if you usually wear glasses, you do not have to wear them for your passport photograph,
but if you do:
•
they must show your eyes clearly (no tinted lenses)
•
no flash reflection off the glasses
•
if possible avoid heavy framed glasses
•
frames should not cover any part of your eyes
•
lighter framed glasses are preferable (if possible).
