Pre-Participation Physical Exams (Sports Physicals)
Wednesday, June 10, 2015 (1:00 - 3:00 pm), in the High School Gym
$25.00 CASH ONLY
For your convenience, Dr. Gregory Cibor, DO FAO ASM (Doctor in Family Medicine and Sports
Medicine) from St. Johns/Providence (and a DCHS alum, class of 1989) will perform on-site sports
physicals at Divine Child on June 10th from 1:00 pm – 3:00 pm. Physicals will be conducted in the High
School gym. These physicals are being offered to all DCHS students.
Dr. Cibor and his staff of residents and nurses will provide a complete physical to include height,
weight, vision screening, blood pressure, orthopedic screening and a cardiac screening questionnaire,
ending with the physician examination. ECG, ECHO tests and any immunizations will NOT BE
OFFERED.
NOTE TO ALL INCOMING 9TH GRADERS: This on-site physical exam will cover
SPORTS ONLY. The mandatory ‘green’ school entry physical for all 9th graders requires
updated immunizations which cannot be performed at this event. It should be noted that
the MHSAA form will still be required for any student wishing to participate in high school
athletics. It is strongly recommended that families of incoming 9th graders request their
personal physician complete both the green form and the MHSAA form during their
student’s appointment
The cost for physicals this year is $25. Cash is the only method of payment accepted.
No appointment is required. The athlete is requested to arrive with payment along with a completed and
signed sports physical form. Any ‘yes’ answers need explanations. The new MHSAA sports physical
