Dear Parent/Guardian:
In an attempt to better serve our student athletes, Tyler ISD will conduct a mass pre-participation
physical exam for the 2015-2016 school year. All students in grades 6-11 interested in participating in
athletics are encouraged to take part in this event. This screening will be provided courtesy of Trinity
Mother Frances Health System on Saturday, April 25 at Trinity Mother Frances Health Park Plaza.
Tyler ISD has initiated a new process of accepting the required athletic participation forms
electronically. We will no longer use a hard copy of the UIL forms. The only hard copies that will be
accepted are the physical exam and medical history forms. Physical exam packets are available online.
Please complete these electronic forms in their entirety and print the medical history and physical exam
forms and turn them in to the athletic trainers by Friday, April 17.
Parents are welcome to transport their student(s) during their school’s allotted time frame. See the
schedule below and the attached map. However, transportation from your students’ campus to and from
Trinity Mother Frances Health Park Plaza will be provided by Tyler ISD.
**All students, regardless of how they arrive at physicals, via parent or bus from their campus,
should be dressed in athletic attire (shorts and t-shirt) to expedite the exam process.**
Saturday, April 25 is the only day Tyler ISD will conduct this free pre-participation physical
exam. Thank you for your attention in helping provide the best care for your student. If you have any

Schedule – Buses will leave the campuses at the listed times and return as soon as all student exams are completed.
Parents who bring their students should have them at Health Park Plaza for check in by the times listed below.
7:30 AM – Hubbard/Moore
8:00 AM – Hogg/Stewart
8:45 AM – Boulter/Dogan
9:15 AM - Robert E Lee
10:00 AM - John Tyler
