ENTRY FORM
Elkhorn Days 2015 is scheduled for June 6, 2015. Family Festival in
the Park at Ta-Ha-Zouka Park will run from noon - 4:00 p.m.
Booth Participants will be able to start setting up your 12’ x 12’ booth at 9 :00 a.m.
with tear-down completed as soon as possible after 4:00 p.m. You must provide
your own canopy, table and chairs. Spaces are filled on a first come / first serve
basis. In certain situations, duplicate entries may be returned. Once accepted, all
fees are non-refundable. If you have questions, please contact Jacob at (402)

***All Booth Entries will be asked to help support our Theme as listed above. The booth voted “Best”
representation of the theme will receive a certificate for a FREE Booth entry in 2016***

The parties to this agreement jointly hold each other harmless and waive all rights of subrogation regarding any and all liability for bodily
injury and Property Damage that may occur during and connected to the Elkhorn Days event.

All entries are subject to the approval of the Elkhorn Days Board of Directors. All information must be completed in order for your entry to
