The Recruitment Process
The recruitment process for this position will involve; submission of your application including cover
letter addressing the selection criteria, resume and completing the application form, a phone
interview, participation in an assessment centre and face to face interview, reference checks, and
completion of a functional pre-employment assessment.
Note: We currently have positions available for the afternoon shift 2:15pm – 9.15pm Monday to
Sunday. Please ensure when completing the application form you indicate your availability.

Context and Purpose
HammondCare is an independent Christian charity specialising in dementia care, palliative care, and
rehabilitation, specialist mental health for older people and other health and aged services. The
organisation provides these services through sub-acute hospitals, residential care and innovative
community services and has a particular commitment to dementia care and research as well as to
older people who are financially disadvantaged.

Position Objectives
The Registered Nurse makes a significant contribution to the care delivery within our facility by
advising and supporting the care team including specialised and dementia carers in providing care
that is aligned with HammondCare’s philosophy of care.
This position is a clinical support role. It is the expectation from HammondCare that this role is carried
out within a consultative and advisory context rather than an authoritarian approach. The position has
a level of professional accountability and responsibility in line with the registration requirements as a
Registered Nurse grade A from the Nurse and Midwives Board of VIC.
The Registered Nurse is a core member of the Caulfield team and works with both the management
of the facility and the S&DCs to ensure resident outcomes are met.
The position works within the philosophy and core values of HammondCare to ensure that the service
is tailored to the needs of the individual clients and delivered in a style that aims to maximise client
satisfaction.








Facilitation of care delivery by the care team including care workers (S&DCs)
Education in clinical areas and processes
Consultative in case management and clinical decision making
Clinical oversight and professional competence as a Registered Nurse
Support and investigate clinical / resident incidents as required
Support families and carers
Implement a palliative approach to dementia care
Communicate effectively across all levels
5.
Key Responsibilities
5.1. Demonstrate and model the Dementia Care Philosophy

5.2. Care Team membership








Communicate with S&DCs to capture issues requiring clinical input. Respond and advise the
S&DCs on a course of action and provide clinical guidance and recommendations
Prioritise resident clinical needs and communicate effectively with the care team and S&DCs
Consult and support the S&DCs throughout each shift and clarify with them any issues of
concern and assist them in completing documentation and developing a plan of care for the
resident
Attend team meetings, when appropriate and actively participate and discuss team issues
and give as well as receive appropriate feedback
Respect all team members and maintain confidentiality
Demonstrate leadership and conflict management skills
Participate in facility meetings / committees
Participate in activities and quality programmes designed to maintain or improve the quality of
care
5.3. Development and maintenance of Case Management through the S&DC role







Support and empower S&DCs in their role as Case Managers
Actively support families,attend case reviews and actively participate in this process with staff
and families
Encourage ownership of care delivery by S&DCs
In collaboration with the S&DCs, ensure that individual plans of care for each resident are in
place and that the S&DC is knowledgeable on how these are maintained, reviewed and
followed
Maintain and review clinical records (e.g. assessments, wound charts, care plans, progress
notes etc.)
Liaise with other health professionals for the provision of the services to meet residents’
needs when applicable e.g. dental, LMOs
Assist S&DC’s act as residents’ advocate and assist individuals and their families to make
informed decisions through the HammondCare case management model
5.4. Technical nursing advice and support









Provide clinical and specialised nursing support and advice as required
Ensure a culture of empowerment exists for S&DCs to work within
Assess, monitor and evaluate work practices of S&DCs and assist them with skill
development as required
Assist in counselling / debriefing staff and provide support to families in critical incident
situations
Ensure the S&DCs know the process for escalating issues that require the input of a
Registered Nurse
Function in accordance with all relevant legislation / competencies
Maintain current professional knowledge and skills for clinical competency
Maintain annual registration requirements with NMB
Take action to rectify any unsafe nursing or S&DC practices
5.5. Other









Act as a resource person and assist the S&DCs by way of discussion (incorporating their
ideas and information as well as your own), on how to deal with behaviours of concern
providing guidance, instruction and leadership when required
With S&DCs and the facility management, participate in ACFI documentation
Monitor the correct use and maintenance of equipment
Provide support with the management of clinical incidents as required, ensure documentation
and investigation is captured in Riskman
Adhere to WH&S policies and procedures, infection control, medication management and
continuous improvement program
Be involved in the audit schedule of the facilities including triggered audits and take
responsibility for actioning areas requiring improvement
Pursue continuous improvement across all levels of the role
Proactive professional development for self
Oversee facility when manager off site

the employee to meet changing organisational needs. The employee will be notified of changes in

Salary Range
$30 - $36.80 p/h depending on experience plus shift
allowances, loadings and salary packaging.

Orientation
A comprehensive orientation and transition program will be provided to all new staff. This program
not only include classroom training and clinical practice development, but also includes specially
allocated time and space for you to get to know the residents and families who have made their
home at the village.

Training Organisation). Annual Development Days and Professional Development Days will be
conducted by HammondCare College and the Dementia Centre, ensuring that all staff are
