Galilee Bible Camp has implemented a policy where we require a reference from a church leader for all staff who wish to serve at
camp. It is extremely important that we receive a thoughtful evaluation of the individual from you. By endorsing the candidate,
Galilee Bible Camp is relying on this information that you are providing.
Please complete the attached and return directly to camp. There are two methods for you to return the form, listed below.

If you are completing the form electronically, save the file when complete. Please include the candidate’s name in the file
name and remember where you save it. Please email the file to camp at [email protected]

Please be aware, the individual that approached you, WILL NOT be allowed to serve if we have not received this document.
