HOPE can GET YOU ON YOUR WAY!

HOPE can GET YOU
ON YOUR WAY!
When the crisis is over, and an individual is
being discharged from the Crisis Response
Center, they may have questions like:
 What do I do next?
 Where will I go now?
 How do I get there?
 Where will I live?
 Can someone help me to understand
the discharge process?
Located in the CRC, the
S.P.A.N. staff can help!
For more information, please call
