National Advance Care Planning Day is April 16th, 2015 Advance Care Planning (ACP) is just another way that you care for yourself – and for those around you. When you do advance care planning, you give others the gift of knowing your future health care wishes, allowing them to speak confidently for you if you couldn’t speak for yourself.¹ The Advanced Care Planning Education Program (ACPEP) for Health Care Providers and Residents of Waterloo Wellington is officially underway! Under this three-year program, standardized ACP protocols, language, and forms that are used by Health Care Providers will be consistent with Ontario legislation and will be promoted to ensure the same practices are used and communicated across the continuum of care. With more information, Residents of Waterloo Wellington can have discussions that will inform their Substitute Decision Maker(s) about their future health care wishes in the event they aren’t able to speak for themselves. Over the next year the ACPEP team will be focusing on engaging the Community and Health Care Providers to determine what is needed as well as how to achieve these goals.
