Mail entry form to: EDBA, PO Box 22565, Southbrook PO, Edmonton, AB, T6W 0C3 or you can drop off the
entry form in an envelope with postage with the post office clerk (post office is inside Sobeys Southbrook
at 111 St and south of Ellerslie Road).
ENTRY FEE MUST ACCOMPANY ENTRY FORM SUBMISSION. PAYMENT ACCEPTED IN CHEQUE ONLY.
Make cheques payable to "EDBA". No post-dated cheques. NSF fee will be charged on bounced cheques.
All entry forms must be received by 5:30 p.m., Apr 6, 2015. Please allow for mailing time.
Players who withdrew from tournament may not receive a refund on entry fees submitted.
EDBA reserves the right to limit the total number of entries accepted on a first-come-first-served basis and
combine, cancel and rearrange events and age groups depending on the number of entries.
