MHS Prom Polices & Contract 2015
Dear Mukwonago High School Families:
Each student wishing to attend Prom on Saturday, May 9th must thoroughly read the policies below and sign
the attached contract. A signed contract must be turned in for a student to purchase a Prom Ticket.






Tickets will be on sale April 20-23rd at MHS
Grand March, May 9th, arrival no earlier than 4 pm at MHS
Busses leave approximately 6 pm from MHS to Olympia
Prom dinner/dance: 7pm – 11:30pm
Return to high school: 12am
Post-prom: 11pm-3:30am
To sit with friends at dinner and to be assigned to the same bus, please paper clip signed contracts together
and turn them in (in groups up to 10) when purchasing your tickets.
To attend the Mukwonago Prom grand march, dinner, and dance, I understand that…

I must turn in a signed contract to purchase a Prom Ticket
I must have a driver’s license or student ID, and a valid ticket, and a signed contract on file to attend Prom
Mukwonago School District policies and consequences are enforced at Prom like all events
If I leave the dance, I may not return. If I invite other students (MHS or non-MHS) who do not have a prom
ticket to Olympia, my parents will be called, and any involved parties will receive a referral to AP.
I cannot get a refund once I’ve purchased a Prom ticket
Neither I nor my date will dance in a way that is inappropriate for a high school event; this includes lewd
dancing and behavior that includes grinding, fondling, disrobing, sexual kissing, and any behavior a chaperone
deems improper or indecent. Chaperones will ask you to leave the dance floor after one warning; chaperones
will contact your parent/guardian and ask them to pick you up from the dance if such behavior continues
I must adhere to the MHS Prom Dress code to enter the grand march, dinner, or dance.
If my date is not an MHS student, I must complete an Prom Guest Pass Application and submit this to the AP
Office for approval no later than April 15th prior to purchasing my date’s Prom ticket—and, it is my
responsibility to ensure that my guest fully comprehends the expectations and rules of MHS
Mukwonago High School Prom Dress Code – Prom is a Formal Event: dresses, dress suits, or tuxes must be worn.
Dress Guidelines:
▫ Dresses may not be cut below the bust line
▫ The bust line continues around your side directly under the armpit
▫ With your arms down by your side, if flesh touches flesh below the bust line, the dress is inappropriate
▫ Dresses may be backless; however, they may not be cut below the natural waist
▫ Dresses may not have the midriffs exposed including both the front and sides
▫ With arms down by your side, if flesh touches flesh in the midriff section, the dress is inappropriate
▫ Dresses may not be shorter or have a slit that exceeds 3 inches above the knee
▫ Dresses may not be constructed with see-through fabrics such as: Tulle, Netting, or Illusion
▫ No pinning will be allowed as an alteration for a dress if without the pins the dress does not meet the dress code
▫ Fabric inserts must be sewn as alterations to the dress
▫ No cover-ups (shawls/wraps, sweaters) will be allowed over dresses that do not meet the dress code
▫ Undergarments should not be visible
Formal Tuxedoes or Dress Suit (Coat/Vest & Tie) Guidelines:
▫ Dress shoes are the appropriate footwear for gentlemen
▫ Shirts must be worn at all times
▫ No Costumes
While we will be reasonable in our interpretation of these guidelines, students and guests dressed inappropriately will not be
allowed to enter or attend Prom. A Dress Code Check will be set up for students and guests to pass through prior to entering
the grand march. Any student or guest inappropriately dressed will be denied entry and asked to leave the premises
immediately without refund.
MHS Student Prom Contract 2015
All persons attending Prom activities are required to have a contract signed by their parents on file
regardless if they have reached age 18. PRINT neatly please.
As a Mukwonago High School student:
I have read the MHS prom policies and dress code, and I understand that all district rules and policies apply at
Prom; and, I will not engage in any inappropriate behaviors; if I violate these policies I will be removed from the
dance without a refund, and I may have future privileges revoked:

*Students signed up for Post-Prom must get off the bus at post-prom; if they change their minds at that point a parent can be
called from post-prom and the student can be picked up.
Turn this page in during lunch hours to Mrs. Iverson or Mrs. Wierzbicki when you purchase your Prom Ticket. Remember
contracts can be paper clipped in groups up to 10 to ensure seating at the same table for dinner at Prom.
