Support is provided to practices to become registered for the related Practice Incentive Payment. www.inwmml.org.au/closethegap This program provides Aboriginal and Torres Strait Islander patients with a chronic disease who are referred by their GP with: 1) Care coordination through qualified healthcare workers; and, 2) Access to Supplementary Services using a flexible pool of funds that can assist patients receiving care coordination. www.inwmml.org.au/closethegap The Accreditation Support Service provides advice, guidance and mock audits to new and existing practices across the region. This support is in accordance with the Standards for General Practices (4th Edition) developed by the Royal College of General Practitioners (RACGP). CAREinMIND™ includes a number of mental health programs, including ATAPS (Access to Allied Psychological Services). It is designed to be used at the point of care and is developed by GPs for general practice, with input from specialists and other health professionals. https://melbourne.healthpathways.org.au/ Immunisation Support Service NPS MedicineWise / Quality Use of Medicines Older Adults Program This service aims to enhance the quality of immunisation services and improve local rates to achieve the national target of at least 90% of children under seven fully vaccinated. Support includes: Data cleaning to improve rates and payments Schedule; NIP vaccine management; cold chain training; and, Fridge temperature logging. www.inwmml.org.au/immunisation Providing an educational visiting program which delivers accurate, independent and practical information on current therapeutic issues to GPs, general practice nurses and pharmacists.
