Aboriginal Health: Closing the Gap Program
Aboriginal Health: Care Coordination and Supplementary Services
Accreditation Support Service
This program aims to increase in the overall health of the Aboriginal and
Torres Strait Islander population; improve access to culturally sensitive
primary care services for Aboriginal and Torres Strait Islander peoples; and,
improve management of chronic conditions experienced by Aboriginal and
Torres Strait Islander peoples. Support is provided to practices to become
registered for the related Practice Incentive Payment.

This program provides Aboriginal and Torres Strait Islander patients with a
chronic disease who are referred by their GP with: 1) Care coordination through
qualified healthcare workers; and, 2) Access to Supplementary Services using a
flexible pool of funds that can assist patients receiving care coordination.

The Accreditation Support Service provides advice, guidance and mock audits
to new and existing practices across the region. This support is in accordance
with the Standards for General Practices (4th Edition) developed by the Royal
College of General Practitioners (RACGP). These standards provide a robust
template for quality care and risk management in general practice.

Australian Primary Care Collaboratives
Through the Enabling System-Wide Advance Care Planning (ACP) Project we
are working to support health care providers to incorporate advance care
planning as part of usual practice. In this project, General Practice can be
supported through quality improvement and access to ACP education and

This program supports general practices and other primary health care
providers to work together to: improve clinical patient outcomes; reduce
lifestyle risk factors; help maintain good health for those with chronic and
complex conditions; and, promote a culture of quality improvement in
primary health care. Targeted support is provided to practices wanting to find
better ways to provide primary health care services to patients through
shared learning, peer support, training, education and support systems.

Chronic Disease Management Service
This program aims to improve quality and patient access to cancer services in
primary healthcare by strengthening stakeholder relationships and increasing
the capacity of primary healthcare providers to provide cancer related

CAREinMIND™ is a free psychological counselling service for those of all ages,
on low incomes. CAREinMIND™ includes a number of mental health programs,
including ATAPS (Access to Allied Psychological Services). Clients receive best
practice services that are clinically governed and tailored to the needs of our

This service provides information, training, education and resources to
improve the management of chronic disease including systems development
and use of MBS items. Support is based on the Wagner Chronic Care Model
and encompasses the National Chronic Disease Strategy set by the Australian

Diversity in Practice
eHealth and Information Management Support Service
This program aims to improve access by vulnerable people to comprehensive
and connected primary health care. The program provides support to
providers on the following areas: Alcohol and other drugs; culturally and
linguistically diverse (CALD) communities; Lesbian, Gay, Bisexual,
Transgender, Intersex and Queer (LGBTIQ) communities; and, those

HealthPathways Melbourne is a free website with locally agreed and
evidence based information on the assessment and management of common
clinical conditions, including referral guidance. It is designed to be used at the
point of care and is developed by GPs for general practice, with input from
specialists and other health professionals.

Immunisation Support Service
NPS MedicineWise / Quality Use of Medicines
Older Adults Program
This service aims to enhance the quality of immunisation services and
improve local rates to achieve the national target of at least 90% of children
under seven fully vaccinated. Support includes: Data cleaning to improve
rates and payments Schedule; NIP vaccine management; cold chain training;

Providing an educational visiting program which delivers accurate, independent
and practical information on current therapeutic issues to GPs, general practice
nurses and pharmacists. This service also supports Clinical Audits and access to
RADAR (Rational Assessment of Drugs and Research), one of the NPS online

Providing information, training, education and resources to all GPs and
practices in our catchment, including: MBS items relevant to RACFs; GP
payments for quality improvement activities; Aged care initiatives; and,
Telehealth. This program also aims to strengthen GP and allied health

targeted support. The service also supports practice managers and GPs to
maximise on the potential of nurses within their practices.

expert guidance and support to general practices in tailoring their services to
better meet the needs of their local communities, thereby significantly
