All applicable federal, state, provincial or other tax liabilities (including but not limited to income taxes)
arising from participation in the Sweepstakes or award of any prize(s) will be the sole responsibility of the
winner(s). Winners will be issued an IRS Form 1099 from the Sponsor(s) or Sponsor’s designee for the
total ARV of the prize won (as stated herein) for the calendar year won. The Sweepstakes administrator is
not the supplier or guarantor of any prize.
Second Chance Drawing: In the event that all First Prizes are not claimed/awarded by the deadline for
Prize Claim submissions, Sweepstakes administrator shall conduct a supplemental second chance random
drawing to award any unclaimed First Prizes, up to a total of twenty (20) First Prizes (“Supplemental
Second Chance Drawing”). If necessary, such drawing will be held at Sweepstakes administrator’s offices
in Burbank, California on or about May 19, 2014 (“Drawing Date”). To enter and be eligible for the
Supplemental Second Drawing, an entrant must legibly hand print his/her First Name, Last Name, Date of
Birth, Street Address (no P.O. Boxes), City, State/Province, Zip/Postal Code, and Phone Number
(including area code) (“Supplemental Second Chance Drawing Entry Information”), on a 3”x5” postcard
and mail to “Wheel of Fortune Millionaire Maker Sweepstakes Supplemental Chance” P.O. Box 7719,
Burbank, CA 91510 by May 16, 2014 (“Supplemental Second Chance Entry”). Supplemental Second
Chance Entries are limited to one (1) per person during the Sweepstakes Period. Multiple Supplemental
Second Chance Entries will be disqualified. Supplemental Second Chance Entry must be filled out and
mailed separately. All Supplemental Second Chance Drawing Entries must be completed in full, be valid
and accurate and original. Proof of submitting a Supplemental Second Chance Entry will not be deemed
by Sponsor as proof of receipt or entry into the Supplemental Second Chance Drawing. Any attempted
form of entry into the Supplemental Second Chance Drawing, other than as described herein is void. No
copied, facsimile or mechanically reproduced Supplemental Second Chance Entries accepted. Odds of
winning a prize in the Supplemental Second Chance Drawing depend on the number of eligible
Supplemental Second Chance Entries received and number of First Prizes unclaimed/unawarded in the
original entry period of the Sweepstakes. If all twenty (20) First Prizes are awarded during the original
Sweepstakes Period, no Supplemental Second Chance Drawing will be held.
Notification: Twenty (20) potential First Prize winners will be notified via telephone by the Sweepstakes
administrator at the phone number provided on his/her Prize Claim or Supplemental Second Chance Entry
if necessary within approximately one (1) week of the Drawing Date. Sweepstakes administrator will
make three (3) phone call attempts within a period of two (2) business days to each potential First Prize or
Supplemental Second Chance Drawing winner. If a First Prize winner or a Supplemental Second Chance
Drawing potential winner, as applicable, cannot be reached after three (3) phone call attempts within two
(2) business days; if he/she is found to be ineligible; if he/she does not comply with these Official Rules;
or if his/her First Prize or First Prize notification is not received in timely fashion for any reason, such
potential winner will be disqualified and an alternate potential winner may be selected in Sponsor’s sole
5
discretion. Potential First Prize winners will be required to sign, execute, have notarized and return an
original unmodified Affidavit of Eligibility, a Liability Release, (where legal) a Publicity Release and any
other documentation that Sponsor and/or the Sweepstakes administrator requires (collectively
“Affidavit/Release”) within two (2) days after receipt by the potential First Prize winner or such potential
First Prize winner may be disqualified and an alternate potential First Prize winner for such prize may be
selected. Travel guests must execute and have notarized (and return within two (2) days of receipt by
winner) an original unmodified Liability Release and a Publicity Release (“Travel Guest Release”).
While faxed and/or emailed Affidavits/Releases and/or Travel Guest Releases have the same force and

Canadian entrant receives a potentially winning Game Piece (or is selected in the Supplemental Second
Chance Drawing as described above), he/she must answer correctly within a specified time a
mathematical skill-testing question without the benefit of any mechanical calculating devices. The test
will be administered by the Sweepstakes administrator at a pre-arranged mutually-convenient time, and
only if the question is answered correctly in the allotted time period will the Canadian entrant become a
potential prize winner. If the question is not correctly answered in the allotted time period, prize will be
forfeited and will be awarded to an alternate prize winner. By returning the completed Affidavit/Release,
First Prize winner attests to the provisions stated therein including the accuracy of the information
contained therein and compliance with these Official Rules.
Million Dollar Event Drawing: During the week of July 14, 2014 (exact date and time to be determined
by Sponsor in its sole discretion and subject to change) while the First Prize or Supplemental Second
Chance Drawing winners or their designated Proxy are at the Event, a random drawing will be conducted
from among all verified First Prize winners to determine the grand prize winner by the Sweepstakes
administrator in Las Vegas, NV. If a Canadian entrant is selected as a potential Grand Prize winner,
he/she may first answer correctly within a specified time a mathematical skill-testing question without the
benefit of any mechanical calculating devices before being declared the Grand Prize winner.
