the date on her order form and fills in the circle for the seating area she wants. She then writes the number and total cost for each type of ticket she wants.
If she wanted to order additional tickets in other seating areas (for her out-of-town friends, for instance), she would use a separate line on her form.

If you’d like to be seated with friends for Auditorium events, but don’t wish
to combine your orders, you may select our Sit with Friends option. You may
have up to four orders seated together. There is an additional $10 service
fee for each order. All orders must be delivered to the Box Office together,
in person, or in one envelope. Please note: Combined orders will be filled
according to the patron with the smallest loyalty point total. Call (812) 8551103 with any questions you may have.

4 Payment Methods
Select ONE method of payment. We are unable to split your payment between multiple

50/50 Payment Option
IU Payroll Deduction
I authorize IU Auditorium to bill 50% of my total subscription order to my credit card at the time the order
is received, and the remaining 50% on August 1, 2015.
Please charge my tickets to IU Payroll Deduction. I am a Bloomington campus faculty or appointed
staff member, and hereby authorize that the deductions be made from my salary until the total cost
is deducted. If employment is terminated, I agree to remit the remaining balance immediately to the
Box Office by check or credit card. NOTE: The cost of your order will be deducted in three monthly
installments beginning in October. Deadline for payroll deduction is August 28, 2015.
