STARTING JANUARY 6, 2015, YOU AND YOUR ENROLLED
SPOUSE/SAME-SEX DOMESTIC PARTNER WILL HAVE AN
EVEN EASIER WAY TO ACCESS PERSONALIZED HEALTH
INFORMATION FROM THE LIMEADE WEBSITE.
For starters, there’s a new URL for all Healthy Rewards participants:
healthyrewards.limeade.com.
If you’re a Kohl’s Associate
You have two simple options for reaching your personalized information:
1. SIGN-IN: Use this option to access your Well-Being Assessment and
other personalized information directly from the Limeade website. See
page 2 for details.
OR
2. TOTAL REWARDS LOGIN: Use this option to access the Limeade
website from yourtotalrewards.com/kohls. You’ll need to enter your
user ID and password for the Kohl’s Total Rewards website and then
click the link to the Limeade website (“Healthy Rewards”) from
the home page.
This approach takes a couple more steps but gives you the convenience
of single sign-on—so you don’t have to track a separate sign-in name
and password for the Limeade website.
NEED HELP WITH YOUR LIMEADE PASSWORD?
If you’re an enrolled spouse/same-sex domestic partner
No worries! Whether you’re a Kohl’s Associate or an enrolled
spouse/same-sex domestic partner, you’ll follow these steps:
You’ll simply sign in as described in Option #1 at healthyrewards.
limeade.com. Once you enter your sign-in name and password,
you’re all set.
• If you’ve already set up a Limeade sign-in name, just
click the Trouble Signing In? link and follow the prompts
to reset. See page 2 for more details.
• If you haven’t set up a Limeade sign-in name or
password in the past, click the Get Started button. See
page 3 for step-by-step instructions.

How to Set Up Your
Limeade Password
If you’ve already logged on to the Limeade website
Follow these steps if you’ve already logged on to the Limeade website—
for example, you’re a current Kohl’s Associate with access to the Kohl’s
Total Rewards website or you’re an enrolled spouse/same-sex domestic
partner and you’ve forgotten your password.

Go to healthyrewards.limeade.com and
click the Trouble Signing In? link.
From the next screen, enter your email address
and click the Send password reset email button.
Be sure to use the email address that’s already
on file with Limeade. You’ll also find answers to
frequently asked questions about password
resets and changes.
You’ll receive an email with a link to another
screen that allows you to create a new
password. Just be sure to click the Create my
password button after you’ve entered and
confirmed your new password.

How to Set Up Your
Limeade Sign-in Name
If you’ve never logged on to the Limeade website
If you’ve never logged on to the Limeade website through the
Kohl’s Total Rewards website or through the current spouse/
same-sex domestic partner URL, you’ll need to follow these easy
steps to set up your sign-in name and password.

Go to healthyrewards.limeade.com and
click the Get started button.
On the next screen you’ll be prompted to enter
your last name, an email address, the last four
digits of your Social Security number and your
birthdate. Click the Find my account button to
move to the final step.
On the last screen, you’ll be asked to choose
your language preference and your sign-in
name. You’ll also set and confirm your password.
Then click the Create my account button and
you’re all set. You can complete your Well-Being
Assessment and start earning points for your
Healthy Rewards points bank.
