How to register
You can register online at: http://lasavictoria.asn.au/education-training/srs-training/#0
You can also use the registration form on the flyer and return it by fax or mail.
If the workshop is fully booked, you will be added to the wait list and alerted of the next available
workshop.
If you do not receive notification of your booking 7 working days prior to the training please contact
LASA Victoria.
Registration and Cancellation Policy
Should any attendee withdraw from the training workshop less than 48 business hours prior to
commencement, or not attend on the day of the workshop, a $30 administration fee may be applicable.
A substitute attendee is welcome to come on your behalf to avoid any administration fees. Please notify
the SRS Training Department at LASA Victoria of any changes in attendance no later than 48 hours
before the workshop.
By forwarding the completed Registration Form you agree to comply with LASA Victoria Registration
and Cancellation policy.
Registration per facility
Each facility can send a maximum of three (3) staff members to each workshop. If you would like
to send more than three staff members please contact SRS Training Department at LASA Victoria to
confirm availability.
Backfill claims
The Department of Health will provide a contribution of $50 to cover the cost of replacing a staff member
attending any of the workshops.
To claim backfill, please complete and send the tax invoice provided to LASA Victoria no later than
three (3) months after the workshop takes place or before the end of the financial year.
Please send individual invoices for different workshops. GST is $0 and needs to be stated on your
invoice. Attendance Sheet must be signed to be eligible for backfill.
Attention: Personal Support Coordinators
Attendance at any of these training workshops may count towards the minimum 40 hours training you
are required to do (see regulation 36(3) Supported Residential Services (Private Proprietors) Regulation
2012). Keep your Certificate of Attendance as evidence.

Please indicate special dietary requirements
on your Registration Form.
On completion of the workshop you will receive a
Certificate of Attendance. Please ensure that your
name is written clearly on the Registration Form so
that your certificate is completed correctly.
Evaluation Forms
Condition of Registration
Your feedback is important to us and we
encourage you to complete an evaluation form
at the end of the workshop.
If you have been requested to attend any of our
workshops as a ‘Condition of Registration’ it is
important that you notify LASA Victoria to ensure
your registration is prioritised.
