Science –science reasoning, science knowledge, biology, chemistry,
anatomy, physiology, basic physical principles, and general science (54
items, 66 minutes)
English and Language Usage – punctuation, grammar, sentence
structure, contextual words, and spelling (34 items, 34 minutes)

back to you and you will include these sealed
observation forms with your completed application

It is valuable for you to discuss with the RDH the
importance of filling out all boxes, sections and
comments areas of the form. This provides maximum
information to reviewers for which they can award
points.

application packet—form is then signed by supervisor/volunteer
