After that— 365 calendar days Selection Criteria: TEAS V. 5 TEAS = Test of Essential Academic Skills Version 5 Required Reading – paragraph comprehension, passage comprehension, and inferences/conclusions (48items, 58 minutes) Math – whole numbers, metric conversion, fractions, decimals, algebraic equations, percentages, and ratio/proportion, stats (34 items, 51 minutes) Science –science reasoning, science knowledge, biology, chemistry, anatomy, physiology, basic physical principles, and general science (54 items, 66 minutes) English and Language Usage – punctuation, grammar, sentence structure, contextual words, and spelling (34 items, 34 minutes) GRAND TOTAL=170 questions in 3 hours & 29 min Selection Criteria: Recommendations One from college or university faculty member One from an employer, co-worker, faculty, advisor or counselor Forms are provided in packet Recommenders give you the forms back in sealed envelopes to place in your application packet Selection Criteria: Observations Combined total of 20 hours Observations take place in two separate clinical settings Observing Registered Dental Hygienists Forms are provided in the application packet You provide form to Dental Hygienist. They will give back to you and you will include these sealed observation forms with your completed application packet It is valuable for you to discuss with the RDH the importance of filling out all boxes, sections and comments areas of the form. This provides maximum information to reviewers for which they can award points. You are required to provide signed proof from doctor/clinic of 2 of 3 Hepatitis B vaccinations completed (one month apart) OR Provide titer showing proof of immunity Optional Selection Criteria: Work Experience 1000+ hours paid chairside Dental Assisting = 10 points OR 250+ hours of Dental Assisting OR Paid Medical Patient Care OR Volunteer Medical Patient Care = 5 points • Student must describe in detail duties performed on form provided in application packet—form is then signed by supervisor/volunteer supervisor, is placed in sealed envelope by supervisor and returned to you to place in your application.
