2015 COBRA INFORMATION BENEFITS COVERAGE TERMINATION Your benefits coverage as an active employee ends on the last day of the month your employment terminates with Stanford Health Care (SHC), including the benefits coverage for your covered dependents. Example: Termination Date 6/30/2015 = Benefits Coverage Ends 6/30/2015 OPTIONS WHEN COVERAGE ENDS COBRA allows you and your covered dependents to continue health plan benefits (Medical/Vision, Dental, Health Care FSA) due to your employment termination from SHC. COBRA PACKET Vita Administration Group (“Vita”) is our Federal COBRA contract administrator. Upon termination, Vita will send you a COBRA Packet to the mailing address we have on file. Expect to receive the packet within 2 weeks of your termination date. Make sure you provide us with a current mailing address and email address for Vita to send the Packet. The Packet cannot be sent prior to termination from SHC. You may elect up to 18 months of continued health coverage if you lose coverage due to termination of employment. COBRA has to be elected within 60 days of the later of (a) the date coverage is lost, or (b) the date the the Election Notice was provided. ADDRESS CHANGE If leaving the State of California or will have a new mailing address immediately following termination from SHC, update your address in SHC Lawson eConnect: Employee Self-Service prior to your termination date. This ensures the COBRA Packet being sent to the correct address. If you fail to update your address prior to termination from SHC, contact VitaCOBRA directly at 650.810.1480 or [email protected]. LAWSON ECONNECT LOGIN To login, use your SID# for the User Name and your SHC network password for the Password. Click on the Globe icon, go to Bookmarks > Employee Self-Service > Address Change. The Effective Date is the date you make the change in Lawson eConnect. You can only access Lawson eConnect while an Active SHC Employee.
