Please check “My.ico.edu” and print the following information:
1. A data sheet with information regarding your next rotation. Please check the sheet to see if
there is additional information you need to pick up or have sent from our office. You may
also wish to ask your preceptor if there is any information you should have received from
them or us when you write your introduction letter.
2. Externship Information form for you to complete and return to the CBE office once your
Summer quarter address and phone numbers are secured. Deadline for this form is
Wednesday, June 3, 2015 to ensure that you receive your mail. Your name must be on
the mailbox at the place you are staying at to receive your mail.
3. Student Time Off information which is to be used if necessary.
4. Academic Calendar 2015-16 for External Clinic Rotations.
5. Sample Introduction Letter.
Please check the “Meditrek” system for the following:
6. Externship Survey and Externship Preceptor Evaluation to be completed at the end of the
quarter by August 5, 2015.
7. Patient Logs which are required to be completed by August 5, 2015, the Wednesday after
your last day of clinic. Your logs will be monitored on a weekly basis. Failure to complete
your logs daily will result in a suspension from patient care until they are completed. All
days missed will need to be made up.

REMINDER: The entire externship survey and externship preceptor evaluation must be
completed by the assigned due date. Your grade will not be released until the patient logs AND the
preceptor evaluation and survey’s are completed. Failure to comply with the requirements by the
deadline will result in a grade change of “I” for the quarter. The ‘I’ will turn into a ‘U’ if not
completed in one week from due date.
Please remind your preceptor to complete your final grade by August 5, 2015 on Meditrek.
