•All exhibitor booth registration fees are
non-refundable and non-transferrable.
•Vendor registration forms received without
payment will not be processed.
•Vendors are responsible for providing
their own table, set up and breakdown.
•Vendor set-up- on Saturday, June 13, 2015
from 7 AM - 9:00 AM.
•All vendors must dismantle their booths
at 4:00 PM on Saturday, June 13, 2015.
•Electricity, Internet and Water services are
NOT included in the exhibitor registration.
NJ Wellness Festival Compliance Agreement
I have read and understand the NJ Wellness Festival
form and agree to abide by the rules as stated. I understand that all promotion of my products and/or services
must take place within the confines of my booth. I further
understand that the NJ Wellness Festival is not responsible
for or liable for damages including, but not limited to, loss
suffered by the Vendor before, during or after the Festival
or as a result of the display of my work equipment, or
materials. The Festival committee reserves the right to
refuse booth space to any group or organization that
does not comply with the rules or regulations of the
State, County, and Township of Warren, NJ.

SCHOLARSHIP FUND, $25 is donated to
