MUNICIPALITY OF NORTH PERTH ELECTRONIC WASTE RECYCLING REQUEST FOR QUOTATION The Municipality of North Perth is inviting quotations from Ontario Electronic Stewardship (OES) recognized processors for the service of electronic waste bins at the Elma Landfill. The Municipality strives to reduce the waste that enters into the landfill and aims to recycle as much as possible in an environmentally responsible way. Quotations will be opened publicly at the Municipality of North Perth, Municipal Office, Thursday, April 9, 2015 at 2:05 pm Local Time. Please provide the following information; 1. It must be issued by your insurer or on a standard CSIO form and signed by an Authorized Representative. In this certificate, the Municipality will require the following: 1. Your full name and address with contact name & phone number 2. Description of your Business Operation 3. Commercial General Liability with a limit no less than $2,000,000.00 4. Tenants Legal Liability, Cross Liability, Notice of Cancellation (30 Days) 5. The Municipality is to be added and shown as an Additional Insured under Liability. 6. If a licensed motorized vehicle is being used, we require Confirmation of Automobile Liability Insurance or Commercial Automobile Liability with a limit no less than $2,000,000.00. Workplace Safety Insurance Board Coverage The Municipality of North Perth will require a current WSIB Clearance Certificate with this quotation. Accessibility for Ontarians with Disabilities Requirements: Prior to the commencement of any work under this contract, the successful bidder will also be required to provide Accessible Customer Service Training to all staff working on behalf of the Municipality of North Perth in compliance with the requirements of the Accessibility for Customer Service Regulation 429/07, Section 6. The Municipality of North Perth will offer assistance with appropriate training resources if required. Health and Safety: The Municipality will expect the successful bidder to abide by all provincial regulations regarding health and safety, as well as licensing and certification. The Municipality of North Perth has a Contractor Health and Safety Package that will need to be completed by the successful bidder, prior to the start of work, which includes the submission of the contractors Health and Safety Policy or the agreement to abide by the Municipality of North Perth’s policies.
