Van Zant, Jr. Superintendent of Schools Greetings To Parents Your child is about to begin a new and challenging experience. There are many things you can do at home to ensure a successful first year at school for your child. Please talk, share, and laugh with your child. Most importantly, READ to your child daily. A supportive home environment will help your child become a successful learner. Kindergarten is an important part of school. The basics taught in kindergarten are foundation blocks for reading and math. Attitudes formed in kindergarten remain with a child for a lifetime. With your help, we hope your child will develop a positive attitude that school is fun, exciting, and challenging. Students entering kindergarten are screened prior to the 30th day of the school year. This screening helps the teacher determine each child’s strengths and plan an appropriate instructional program. We look forward to working with your child in kindergarten this year. LEGAL REQUIREMENTS To enter kindergarten for the 2015-2016 school year a child must be five (5) years of age on or before September 1, 2015, verified by a birth certificate. (Florida Statute 1003.21) Clay County policy has no provision for early admission to kindergarten. Florida’s Compulsory Immunization Law requires that, prior to entry, attendance or transfer; all children comply with the state adopted immunization schedule (Florida Statute 1003.22). For the 2015-2016 school year, DH Form 680 (or Religious Exemption DH Form 681) is the only acceptable immunization certificate for admittance to a public or non-public school, grades Pre-K through 12. The Form 680 can be obtained from your doctor or the Clay County Health Department. The Form 681 can be obtained from the Clay County Health Department. A kindergarten child may be certified fully immunized if, the following vaccinations are completed prior to entry: • • • • • Five doses of Diphtheria, tetanus, and Pertussis (DTaP) vaccine (4 DTaP if 4th dose given on or after 4th birthday) Four doses of Polio vaccine (3 Polio if 3rd dose given on or after 4th birthday) Two doses of Measles, Mumps, and Rubella (MMR) vaccine Three doses of Hepatitis B (Hep B) vaccine NOTE: Hepatitis-B series requires six months to complete. Two doses of Varicella vaccine. Not required if the child has a documented history of chicken pox on the DH680 immunization record. All “initial entry” students to a Florida school (pre-school through grade 12) must present certification of a schoolentry health examination performed within 12 months prior to the first day of school. (Florida Statute 1003.22) The required forms for immunization certification and school-entry health examination must be filed with the school prior to the child entering on the first day of school. In addition to health documentation, you will need proof of date of birth and 3 proofs of residency. Items you may use to prove date of birth are: birth certificate, passport, or life insurance policy. Items you may use for proof of residency are: (2 required) current utility bills, mortgage or rental agreement, plus (1 required) driver’s license, auto insurance, credit card statement, bank account statement.
