Any child who will be 5 on or before September 1
WHAT ?
Kindergarten Round-Up
WHEN ?
Wednesday, May 13, 2015 at 2:00 p.m.
HOW ?
Report to your zoned school with:
(Duval, Lake Forest, Metcalfe, Rawlings, Shell, and Waldo at 3:00 p.m.)
 Birth certificate
 Immunization record
 Proof of physical exam
 Social Security Card
 Two different proofs of residence in parent’s name
Examples of Proof of Residence include:
Property Tax Statement
Declaration of Domicile
Homestead Exemption
Utility Bill
Lease Agreement
If you are unable to attend Round-Up, you may register
your child for kindergarten any time after May 13,2015.
