The Mini External Fixator Surgical Technique 2 The Mini External Fixator – Surgical Technique Indications, Features and Components Indications • • • • • • Complex fractures of phalanges and metacarpals Displaced intra-articular fractures Segmental bone loss Open fractures not allowing stable internal fixation Fractures with associated complex soft tissue injuries Tumour resections Features • • • • Module fits in the Modular Hand System Low clamp profile Compatible with the small External Fixator 107.982 Choice of threaded Kirschner wires of 1.25mm or 1.6mm dia. with corresponding clamps • Radiolucent Carbon-Fibre Rods • Allows sequential clamping of two Kirschner wires in the clamp Components 1.25mm 1.6mm Clamp Bodies for 1.25mm and 1.6mm wires Allow sequential tensioning of Kirschner wires to reduce the operation time. The clamp-body nuts are captured. Slender design contributes to overall low-profile frame for improved patient comfort. Connecting Clamp, 3.0/3.0mm dia. Allows connection of 3.0mm Carbon-Fibre Rods for the treatment of displaced intra-articular fractures. An additionally available 3.0/4.0mm Connecting Clamp allows bridging to the small Synthes® External Fixator for the treatment of complex hand and distal radius fractures. 3 The Mini External Fixator – Surgical Technique Carbon-fibre rods Lightweight for patient comfort. Radiolucent for improved intra- and postoperative radiographic visualization. 1.25mm 1.6mm Threaded Kirschner wires with trocar tip, in two sizes Provide more stability and better hold in cortical bone. Eliminate need to predrill, for reduced operative time. 1.25mm and 1.6mm diameters optimise adaptation to fracture patterns and patient anatomy. Angle Allows lateral application of pins while maintaining a dorsal frame construct, as determined by the fracture pattern and surgeon’s preference. 4 The Mini External Fixator – Surgical Technique Surgical Technique 1 Insert first Kirschner wires Insert one threaded Kirschner wire through the double drill sleeve 1.25/1.6 in each fragment. Place the first two Kirschner wires as close as possible to the fracture in each fragment. 2 Add clamps and rods Connect the appropriate clamp to each Kirschner wire through the open hole of the clamp: • 1.25mm Kirschner wires – Clamp Body for 1.25mm wires • 1.6mm Kirschner wires – Clamp Body for 1.6mm wires Insert a 3.0mm rod to connect the clamps. Reduce the fracture. Lightly tighten the nuts of both pin and rod clamps to provide sufficient stability for radiography and final fracture reduction. Kirschner wire through open hole 5 The Mini External Fixator – Surgical Technique 3 Insert remaining Kirschner wires Insert the two remaining Kirschner wires as far as possible from the fracture, using the remaining clamp holes as guides. K-wire through closed hole 4 Secure all clamp nuts Tighten the nuts on both clamps, using the clamp holder to avoid fracture displacement. Cut the Kirschner wires, if necessary. 6 The Mini External Fixator – Surgical Technique Set contents 115.985 Mini External Fixator Instrument and Implant Set 304.564 Module for Mini External Fixator Instruments and Implants, without contents Item no.
