If you wish to be placed on our Wait List, please print out, complete, and return this Application.
Thank you for your interest in Otterbein Swim Club. To place your name on the Wait List for a
Membership, please (1) complete and sign this Wait List Application and (2) return it to the Swim Club with
a Wait List Application Fee of $50 (payable to “Otterbein Swim Club”). This Fee is not refundable unless
you are not offered a Membership for 2015, 2016, 2017, or 2018 (or you move from the neighborhood
before you are offered a Membership). The Club, for any reason, may elect to return the Fee and modify its
Wait List policies.

Your $50 Wait List Application Fee will be applied to your $500 Initiation Fee. If you are offered a spot for
2015, 2016, 2017, or 2018 and you do not follow through and complete and sign your Membership
Application by the March 15th due date and pay the Annual Dues by the April 15th due date, your place on
the Wait List (and the $50 fee) will be forfeited. (Annual Dues for new Members joining in 2015 are
$800/Family, $550/Couple, and $300/Single and likely will increase by 2018.) If you have any questions
