Confined Space Hazard.
Being aware of some of the most common workplace hazards can help you to recognize and identify
them faster. Some hazards are “invisible hazards”, and that means they aren’t obvious to us at the
first glance. Within a confined space these hazards require close attention. A confined space is any
area with a restricted entry/exiting (a tight space that is not designed for continuous human
occupancy). One common cause of injury is being overcome by toxic gases or having a lack of
oxygen while working in a confined space. A confined space must be tested for hazardous
atmospheres and properly ventilated before workers enter. Toxic fumes can come from hot work
and fill a confined space quickly. Sometimes the materials used at the worksite are discharged into
the air and become airborne contaminants. Keep in mind that decaying matter like garbage and
vegetation can produce toxic fumes as well. All confined space incident must be investigated.
Incident investigations are not conducted to affix blame. They are conducted to find facts to help
prevent recurrence. Almost every incident is the result of a combination of causes. Remember that
the primary purpose of investigation is to identify these causes so that corrective action can be
taken to prevent a recurrence of the incident. Additionally, information collected will be valuable in
meeting the government agency’s reporting requirements. Investigations should be conducted by
the supervisor in charge of the area and/or personnel involved. The investigator must review every
incident report to ensure that appropriate corrective actions take place.
