This questionnaire raises questions relating to your personal and financial affairs. It also seeks
to ascertain the matters that are important to you and which need to be considered when
preparing your Will. We appreciate that you may not be able to answer all the questions. You
may however find it helpful to read all the questions through as the issues raised are ones that
you should be thinking about when you prepare your Will. If you are not able to provide full
details of your assets when completing the questionnaire, it would be helpful if you could
provide at least an estimate of the assets in your estate. If when completing this questionnaire
there is insufficient space to provide all the information, please provide the information on
additional pages as necessary.
