SUGAR ACHIEVEMENT AWARD - Program Code: UEAO526 ($5000; min CGPA 2.00) TO SFU faculty*, staff*, students who meet the following criteria: winner of national or international competition, or recipient of national or international prize or award. REQUIREMENTS: Demonstrated history of leadership in nominee’s field, and accomplishments directly related to responsibilities and activities at SFU. C.D. N ELS O N M E MO RI AL P R IZ E - Program Code: UEAO019 ($500; min CGPA 2.00) AWARDED to a current or retired faculty* or staff* member, or to a current student who has made an outstanding contribution to SFU other than normal or academic work. The C.D. Nelson Memorial Prize is valued at approximately $500 for the purchase of a Work of Art. Upon presentation of a receipt, the recipient will be reimbursed for the value of the award. TERRY FOX GOLD MEDAL AND PRIZE - Program Code: UUAO001 (Gold medal and award of $1000; min CGPA 2.00) AWARDED annually to any student who has demonstrated those personal qualities of courage in adversity and dedication to society which have been exemplified by Terry Fox and his Marathon of Hope. The award includes a cash prize of $1000 and three terms’ tuition upon registration at SFU. REQUIREMENTS: The application or nomination should include appropriate supporting documentation. LAURINE HARRISON UNDERGRADUATE SERVICE AWARD - Program Code: UEAO545 ($1500; min CGPA 2.00) TO undergraduate students in any faculty on the basis of demonstrated leadership and/or service at SFU. The student’s leadership or service may be to the student community, (demonstrated by service to the Student Society, Board of Governors, Senate, or some other University body on which students are represented) or by advocating for students in the community at large (as demonstrated by fundraising activities, or service to off-campus student advocacy groups). REQUIREMENTS: Applications must include a letter from the student outlining their leadership activities and/or volunteer service as well as letter(s) of recommendation confirming these activities. ROGER G. WELCH ALUMNI PRIZE - Program Code: UEAO172 ($1100; min CGPA 2.00) TO an alumnus/alumna of SFU pursuing a degree program or a post baccalaureate diploma. The prize will honor or recognize students who have demonstrated leadership, citizenship and dedication in service to the University community. Participation in the wider community will also be considered. REQUIREMENTS: The granting of the prize will be based on evidence submitted by the applicant or by another person, group or association. ROSSLYN AND MARY PENNEY COMMUNITY SERVICE AWARD - Program Code: UEAO548 ($1000; min CGPA 2.00) GRANTED on the basis of good academic standing and leadership and/or service at SFU to an undergraduate student in the Faculty of Arts and Social Sciences in his/her second, third or fourth year of study. REQUIREMENTS: Preference will be given to students with disabilities who experience issues accessing the physical environment. Application packages must include a résumé outlining the candidate’s leadership activities and volunteer service as well as letter(s) of recommendation confirming these activities. page 1 of 2 ALUMNI ASSOCIATION OUTSTANDING STUDENT LEADERSHIP AWARD - Program Code: UPAO167 ($2000; min CGPA 3.50) TO an undergraduate student in any faculty in his/her third or fourth year with a minimum of 30 units completed at SFU, and demonstration of a combination of outstanding academic achievement and outstanding performance or leadership in another endeavour at SFU or in the broader community. The achievement may be in athletics or the arts, in service to the University or to the community at large. REQUIREMENTS: Nominations, including a letter and résumé from the nominee as well as a supporting letter from an individual who can speak to the achievements of the Nominee, should be sent from the Chair or Director of the nominee’s department or school or from the Registrar. Presentation of the award will take place at the annual Outstanding Alumni Awards Ceremony. Funds will be disbursed upon receipt of funds from the Donor. TD INSURANCE MELOCHE MONNEX OUTSTANDING STUDENT LEADERSHIP AWARD - Program Code: UPAO199 ($1000; min CGPA 3.50) TO an undergraduate student in any faculty who is in his/her third or fourth year with a minimum of thirty (30) units completed at SFU, and has demonstrated a combination of outstanding academic achievement and outstanding performance or leadership in another endeavour at SFU or in the broader community. The achievement may be in athletics or the arts, in service to the University, or to the community at large. REQUIREMENTS: Applications must include a letter and résumé from the student and a supporting letter from an individual who can speak to specific achievements. Funds will be disbursed upon receipt of funds from the Donor. LOUISE BERTHE SAMSON UNDERGRADUATE AWARD - Program Code: UEAO060 ($600; min CGPA 3.00) TO a third or fourth year undergraduate student in any faculty who is doing volunteer work or a work project related to the applicant’s field of study in a recognized charitable or community organization. The successful applicant should have a cumulative GPA of at least 3.00. REQUIREMENTS: The application should include a letter outlining the applicant’s chosen course of studies, a résumé, an explanation of the volunteer or work project activity, and its impact on the applicant. There can be no more than a one term gap between the volunteer or work project activity and the awarding of this Award. LIVING PERSONAL TRUTHS AWARD - Program Code: UPAO195 ($400; min CGPA 2.00) TO a student in any faculty who has demonstrated a significant contribution to reducing discrimination and/or increasing awareness of sexual orientation and gender diversity. REQUIREMENTS: Applications must include a letter and résumé from the student and a supporting letter from an individual who can speak to the student’s achievements. Funds will be disbursed upon receipt of funds from the Donor. WEST COAST COALITION FOR HUMAN DIGNITY COMMUNITY AWARD - Program Code: UEAO201 ($400; min CGPA 2.00) TO a student in any faculty based on demonstrated commitment to and leadership in opposing bigotry and advancing human rights through work in schools, community or non-governmental organizations that work to eliminate racism, sexism, xenophobia, and/or homophobia, or provide services to victims of such.
