Request for Hazlewood Exemption Please read carefully At this time, you may apply for exemption for summer - fall 2015 & spring 2016. Only apply for the semesters you plan to enroll. Read TVC website for more information. You must meet eligibility requirements to receive Hazlewood tuition exemption. Your deadline to apply for Hazlewood is the last class day of the semester for which you are requesting exemption. It is preferable that you apply prior to the census date (12th class day in fall and spring semesters and 4th class day in the summer semester). You must be admitted and must pay your enrollment deposit before you apply. If you have not been admitted, eligibility will not be determined, and the forms will not be retained. Please note that Hazlewood exemption only applies to classes for which the University receives state support. If you are applying for Hazlewood for the first time at this University, you must complete o TVC-ED-1: Texas Hazlewood Act Exemption Application o Statement of Understanding You must specify the semester(s) for which you are applying for exemption, including each semester and year. Example: If you are applying for two semesters, write “summer 2015 and fall 2015”; or “summer 2015, fall 2015, spring 2016”. Packets must be complete and submitted only once with all supporting documents as indicated. Significant delays are possible if applications are incomplete. You must write your UTEID on every page. You must complete your registration and secure your classes by your tuition deadline and ensure your registration is complete and your courses are secured. If you do not take this step, you will lose the classes you registered for. Child (dependent)Documents required if you are applying for Hazlewood Legacy tuition exemption because the veteran is transferring unused hours to you, the dependent child. 1. You must complete Texas Hazlewood Act Exemption Application TVC-ED-1. 2. Provide proof to show that the veteran lives in Texas (a copy of the most recent utility bill with the veteran’s name and address is preferred, a valid driver’s license is acceptable). Veterans who have continuously used Hazlewood exemption since fall 2011 do not need to submit proof of residence. 3. If the veteran served after 9/10/2001 and transferred unused Post 9/11 GI Bill months to you, provide proof of your eligibility or ineligibility for Post 9/11 GI Bill from the Department of Veterans Affairs. If not, disregard. 4. Provide proof of service of veteran: DD 214 and type of discharge. 5. You and the veteran must register at Hazlewood online and print proof of number of Hazlewood tuition hours exempted. 6. You must provide a copy of your birth certificate or recent IRS transcript to show proof of your relation to the veteran. 7. Statement of Understanding 1. You must complete Texas Hazlewood Act Exemption Application for Continued Enrollment TVC-ED-2. 2. Provide proof that the veteran lives in Texas (a copy of the most recent utility bill of veteran is preferred; a valid driver’s license is acceptable). Veterans who have continuously used Hazlewood exemption since fall 2011 do not need to submit proof of residence. 3. If the veteran served after 9/10/2001 and transferred unused Post 9/11 GI Bill months to you, provide proof of your eligibility or ineligibility for Post 9/11 GI Bill from the Department of Veterans Affairs. You must complete Texas Hazlewood Act Exemption Application TVC-ED-1. 2. If you served on or after 9/10/2001, provide proof of eligibility or ineligibility for Post 9/11 GI Bill or Vocational rehabilitation benefits (Chapter 31) from the Department of Veterans Affairs. If you are using Montgomery GI Bill-Active duty (Chapter 30), or REAP (Chapter 1607) submit proof of remaining entitlement. 3. Provide proof to show that you live in Texas (a copy of the most recent utility bill with the veteran’s name and address is preferred; a valid driver’s license is acceptable). 4. Provide proof of service DD 214 and type of discharge. 5. Register at Hazlewood online and print proof of number of Hazlewood tuition hours exempted. 6. Statement of Understanding. If you served on or after 9/10/2001, provide proof of eligibility or ineligibility for Post 9/11 GI Bill or Vocational rehabilitation benefits (Chapter 31) from the Department of Veterans Affairs. If the veteran served after 9/10/2001 and transferred unused Post 9/11 GI Bill months to you, provide proof of your eligibility or ineligibility for Post 9/11 GI Bill from the Department of Veterans Affairs. If not, disregard. 3. Provide proof of service of veteran: DD 214 and type of discharge or DD-1300 if service member died prior to discharge 4. Provide proof of veteran’s disability/unemployability or death due to service related injuries (letter from the Department of Veterans Affairs). 5. You must register at Hazlewood online and print proof of number of Hazlewood tuition hours exempted. 6. You (Child) must provide a copy of your birth certificate or show other proof of your relationship to the veteran. Spouses must provide a copy of their marriage certificate to show relationship to veteran. 7. Statement of understanding. Continuing benefits at the University of Texas at Austin 1.
