How to obtain a Verizon Wireless Mobile Phone Using the FOP Corporate Account Discount Plan If you want to move your existing Verizon cell phone service, or move from another phone carrier, to the FOP’s corporate account, please use the ASSUMPTION OF LIABILITY (AOL) form. The form is in a Word document format. The document can be downloaded at: www.azfop.com/verizon or http://lodge2.azfop.com/files/2012/04/FOP_AOL_JR_POOLE_form.doc Keep in mind, if you move to the FOP’s Verizon corporate account, there are a few things that are different from a personal Verizon account. Most importantly, the rate plans, depending upon the calling plans selected, may result in a savings; however, the corporate plans are not the same plans available through a noncorporate plan. For example; the Share Data Plan isn’t available under the corporate plan. Basically, the corporate share plan shares talk minutes, however, it does not share data. See the Verizon Rate plans at the bottom of this document or at www.azfop.com/verizon . Corporate account members can’t go to a Verizon store to make changes to a plan, purchase new equipment or get assistance with equipment. You must use your My Verizon corporate web account, which you must ask the FOP point of contact to set up 48 hours after your corporate account is activated. Because transferring to a corporate account changes the liability associated with your phone service; o You must remain current in your payments to keep your phone service active. o Equipment and other services (such as downloaded smartphone apps) must be paid with a personal credit card instead of being added to your Verizon bill. After activating your FOP Verizon corporate account, expect to get a final bill for the personal account you are leaving listing all of the charges up to the date of transfer. This final bill must be paid. The final bill also serves as a notice of transfer. The SPOC (Points of Contact) J.R. Pool and Lorraine Thompson are not Verizon employees; they are FOP members who have volunteered to process the paperwork and set up your Verizon Web access. Because she is retired, Lorraine Thompson, Tucson Lodge 1 [email protected] has more time to process paperwork. J.R. Pool, Phoenix Lodge 2 [email protected] is the SPOC for most of the Phoenix FOP members. If you want to move your existing non-Verizon cell phone service to the FOP’s corporate account, before doing so, call your current cell phone service provider to ensure you will not incur an early cancellation fee. If you will incur a fee, determine if it’s something you are willing to pay or you may decide to wait until your contract with your current provider has ended before moving to Verizon Wireless. If your existing Verizon account is Bundled with Century Link or any other type of Bundled payment plan you must de-enroll your Verizon account before it can be transferred to the FOP Verizon discount program. If you do not have a cell phone account with any provider, fill out the AOL form. Since you will not have existing Verizon account information, leave those boxes blank. How to obtain a Verizon Wireless Mobile Phone Using the FOP Corporate Account Discount Plan - Continued The Following is a STEP-BY-STEP procedure for completing the AOL form. Follow the directions posted in each box. Please note some of the information requested may not apply to you. Please do not go into or call a Verizon store for help or questions. They will not be able to help as they not familiar with corporate pricing and have no access to this information Account Information – Relinquishing Customer UNDER THE HEADING ACCOUNT INFORMATION – RELINQUISHING CUSTOMER: IN THE BOX– Signed: Type your name. When Lorraine or J.R. receives your copy, she will verify you are an FOP member and send your paperwork to the Government Business Center for processing- This will take between 3-5 business days- Verizon will send you an e-mail notifying you your request has been completed. Don’t be concerned unless you receive an email stating your application is missing a piece of vital information. If that is the case, provide whatever missing information requested. Once Verizon has sent notification to you that your account set up has been completed, We strongly recommend setting up your My Business account 48 hours after your account has been transferred!! You may receive a greater discounts by ordering phones and accessories thru My Business than you would by calling our Government Business Center. You can sign up for online access through Lorraine Thompson 48 hours after your transfer account is complete- Please email her at [email protected] .
