You are invited to join the Arizona Medical Association for our 2015 ArMA Annual
Meeting of the House of Delegates and President’s Awards Banquet; May 29-30 at
ArMA headquarters, 810 W. Bethany Home Rd., Phoenix. The banquet to be the
evening of the 29th at Phoenix Country Club 2901 N. 7th St.
Please join us at our annual meeting and banquet where you will be provided with the
opportunity to network and visit with physician leaders from throughout Arizona. These
physicians represent the various Arizona county medical societies, the specialty
medical societies, and graduate medical and residency programs.
Register today as an exhibitor/sponsor for a chance to reach a concentrated group of
physician leaders interested in your products and services. You will also make new
contacts and generate leads, while generally solidifying customer relationships by
demonstrating your support. The individual physicians represent approximately 4,600
ArMA physician members.
Your company will be highlighted in the opening announcements and exhibitors will be
introduced during the House of Delegates Meeting. Gold Sponsors will also be
introduced during the President’s Awards Banquet.
Don’t wait to register! Due to ArMA facility size, space is limited and only a select group
of exhibitor/sponsors are invited for the annual meeting. The final deadline for all
materials is April 29, 2015.

LETTER OF AGREEMENT

FEES AND EXHIBIT/SPONSOR SUPPORTER RULES
1. Exhibitor/Sponsorship fee is due and payable in full by April 29, 2015 (Tax #86-0095708) in order
to be included in the printed acknowledgements. This includes one 6-foot table and basic
electrical hookups.
2. Each Exhibitor must provide a gift ($100 minimum value) for drawing on final day of meeting.
3. Only one corporation/organization for each table top exhibit space.
4. All displays must be table top. All exhibits must be contained within the exhibit space. All displays
must be designed so that they will not obstruct the clear view of nearby exhibitor/sponsors.
Exhibitor/Sponsors may not conduct any demonstration or activity which results in obstruction of
aisles or prevents ready access to neighboring exhibitor/sponsors.
5. No hazardous material may be stored or on display in the exhibit area.
6. Management reserves the right to prohibit and/or evict exhibitor/sponsors whose presentation is in
any manner deemed by management as offensive. In the event of such prohibition or eviction,
management shall not be liable to exhibitor/sponsor for refund of exhibit/sponsor fees.
HOLD HARMLESS AGREEMENT
The exhibitor/sponsor assumes the entire responsibility and liability for losses, damages and claims arising out
of injury or damage to exhibitor/sponsors’ displays, equipment, and other property brought upon the premises
of the Arizona Medical Association headquarters and shall indemnify and hold harmless the Arizona Medical
Association and any authorized representative, agent or employee of the foregoing of any and all losses,
damages and claims.
The exhibitor/sponsor agrees to abide by all requirements of the Letter of Agreement.
CANCELLATIONS
Written cancellations received fourteen (14) days preceding the conference will be refunded less a $100.00
administrative fee. No refunds will be made on cancellations received after that time.
