for your life after work. Follow these steps to enroll:

Cedars-Sinai needs to be able
to contact you and send you
information about your pay and
benefits throughout the year,
or you’ll miss out on important
updates. Alerting the post office
or benefit vendors alone is not
enough – you must also change
your contact information (and
address) directly with us.

Medical benefits for yourself (and family).
Cedars-Sinai pay for most of the premium –
you pay only a small portion through payroll
deduction; to ensure that all employees
(regardless of income) have access to
coverage, premiums are based on your pay
rate of $[XXX.XX] per hour.

yourself; this is the ONLY time you can
purchase up to 2x pay in life insurance

Address Phone Change Form.
If at work, you can complete and
submit the form online; otherwise
you must print and send it to the
HR Employee Record Department
